Our dedicated Support Coordination team is here and ready to help you every step of the way on your NDIS journey!

You’ll find Briony in the office Monday, Tuesday, Thursday and Friday, Doreen here Tuesday, Wednesday and Thursday, and Roz in Monday, Wednesday and Friday — ensuring friendly, reliable support throughout the week.

Our door is always open at 123 Curlewis Street, Swan Hill, and we genuinely love when our clients come in to visit — the kettle is on, the coffee is ready, and you’re always welcome!
Whether you need guidance, advocacy, plan support or simply someone to walk alongside you and help make sense of your NDIS plan, we are here for you.

✨ Welcome Back & Happy New Year! ✨

The team at Your Support Coordination is excited to be back and ready for a new year ahead. Based in Swan Hill and Cohuna, we proudly support participants across the region, including the Swan Hill Rural City Council, Gannawarra Shire, Buloke Shire, and Loddon Shire.

As we step into a new year, we look forward to continuing to support our valued clients throughout their NDIS journey, helping you navigate your plans, build strong supports, and work towards your goals with confidence.

We are also always welcoming new clients and would love to support more individuals and families across our local communities.

As the year comes to a close, we would like to extend our sincere thanks to all of our valued clients and the dedicated services we have had the privilege of working alongside this year. Your trust, collaboration, and support have meant so much to us and have made this year truly rewarding.

It has been an honour to work with you and to be part of your NDIS journey. We are grateful for the strong partnerships we’ve built and the positive outcomes we’ve been able to achieve together.

As we celebrate the festive season, we wish you and your loved ones a very Merry Christmas, a safe and relaxing holiday period, and a Happy New Year. We look forward to continuing our work together and can’t wait to reconnect and collaborate again in 2026.

Please note that our Swan Hill and Cohuna offices will be closed from December 23rd and will reopen on January 5th.

Our Brighter Day fund-raising event is happening this coming Saturday the 17th!

Pop down to Ohana House for food, face painting, our raffle and just overall GOOD VIBES 😎

We will be advertising the raffle prizes in the coming days on our socials so keep your eyes peeled (if you want to purchase tickets and can't come to the event- swing us a message!)
