About your role:
As our Support Worker, you will play a key role in empowering our participants to be part of the community, and to live independently. When supporting our participants with domestic assistance, social supports or personal care, your care is person-centric to ensure that our customers live the life they want and choose. This role will definitely gauge with your playful, compassionate, as you shape and improve the lives of our participants.

Key Responsibilities:
Support participants to develop and maintain independence in personal care, health and hygiene (meal times, dressing, toileting and bathing)
Assist with domestic duties such as cooking, cleaning and other activities of daily living
Assist with transportation to day programs, appointments or community access.
Please note: Flexible to work across all Sydney area

About You:
You need to be passionate about making a difference to the lives of others. You pride yourself on your exceptional customer service and verbal and written communication skills. Having previously worked within the Disability sector or a related field, you have knowledge and experience working with mental health. You are highly personable and have the ability to engage people in skill development activities such as social and recreational activities. Most importantly, you strongly align with our mission and values.

You will be/have

Fully covid-19 vaccinated.
Be available for a minimum of 3 days per week.
Availability to work flexibly if needed.
Willingness to travel.
Industry experience and/or personal experience supporting people with challenging behaviours is highly regarded.
Comfortable with providing personal care.
Current Australian Driver's License (Minimum P2 License +) and your own motor vehicle.
First Aid Certificate or willingness to obtain.
Strong desire to support people with a disability to increase and maintain independence and provide a positive outlook in life.
Cert III in disability or relevant industry experience is highly regarded.
Be able to speak second languages, Vietnamese preferable.

SUPPORT COORDINATOR and PSYCHOSOCIAL RECOVERY COACH / Part-time
(20 - 25 hours per week)

The Support Coordinator plays an integral role for NDIS participants in interpreting their NDIS plan, assisting them to establish the supports in their plan and develop their capacity over time to coordinate and manage their own supports.

POSITION RESPONSIBILITIES

Support Coordinator responsibilities:
• Working with participants to achieve their goals in line with their NDIS plan
• Providing ongoing support coordination services to customers in line with their NDIS plan
• Using a coaching approach to assist participants in capacity building
• Maintaining accurate and up to date case notes via Brevity.
• Ensuring Work Health and Safety guidelines are adhered to
• Engaging with participants, collaborating with their supports and 3rd party organisations in a professional and collaborative manner
• Undertaking work in line with the organisations policies, procedures, and values
• Complying with all relevant legislation and code of conduct
• Maintaining an up to date knowledge of the NDIS system and practices
• Maintaining clear professional boundaries with participants
• Maintaining an up to date knowledge of local services and organisations participants’ can link too
• Providing participants with options for services and allowing for choice and control.
* Support coordinator requires experience in NDIS setting and must hold appropriate qualification in social work or mental health.

Psychosocial Recovery Coach responsibilities:
• Assistance for participants to build capacity and resilience to live a full and contributing life
• Work collaboratively with participants, families, carers and other services to identify, plan, design and coordinate NDIS supports
• Support clients to foster hope, identify and develop goals and work together towards achieving goals set out
• Build capacity to develop strong and respectful relationships
• Support to motivate the participants to have increased choice and control and increase their social and economic participation.
• Psychosocial Recovery Coaches require lived and/or learnt experience. They must hold tertiary qualifications in peer work or mental health (minimum of Cert IV in Mental Health Peer Work or Cert IV in Mental Health) or equivalent training; and/or a minimum of two years’ experience in mental-health related work.

ESSENTIAL ROLE REQUIREMENTS
• Hold a current Driver's License
• A road worthy vehicle with comprehensive insurance
• Hold a current Working with Children Check for paid employment
• A Certificate of Completion of Worker Orientation Module
• NDIS Worker Screening Module
• Australian or New Zealand citizenship or a relevant working visa

SKILLS
• Person-centred approach to service delivery
• Effective communication skills
• Ability to work with external stakeholders
• Ability to place the client at the forefront of every decision
• Good administrative skills
• Good understanding of the NDIS
• Good understanding of the community sector
• Good understanding of relevant privacy and confidentiality in relations to participants and their information
