My response in defense of Doulas to todays SMH article https://www.smh.com.au/topic/medical-misogyny-6h6i

I was deeply interested to read the recent discussion in the Sydney Morning Herald around medical misogyny and women’s experiences of maternity care. As a birth and post-natal doula and educator, I would like to highlight the essential role that doulas play in counter-ing many of the very issues raised in your piece.

First, the article outlines how too often women feel unheard, dismissed or controlled in the maternity system. Doulas provide a powerful antidote to that dynamic. When a woman engages a skilled doula, she gains a consistent, non-judgmental advocate whose sole focus is on her priorities, her voice, her choices. This helps to reduce feelings of powerlessness and supports the woman in becoming an active participant in her own birth experience; not simply a passive recipient of care.

Second, your piece highlights how structural issues of gendered authority and lack of shared decision-making can harm women’s health and wellbeing. Doulas contribute to culturally sensitive, relational care that helps bridge the gap between hospital processes and the human being at the centre of them. We don’t replace midwives or doctors; we complement them. The doula’s presence helps all professionals involved to focus more on the woman’s story, her emotional needs, and the bigger picture of her birthing rights.

Third, doulas contribute measurably to better outcomes (fewer interventions, shorter labours, higher satisfaction). When women feel respected and supported, the whole system benefits. In this way, doulas are neither frivolous nor optional—they are evidence-based partners in promoting safe, respectful, rights-based maternity care.

Finally, the article’s theme of misogyny in healthcare demands that we examine our models of care and listen to women’s lived experience. By training doulas and working with birthing families, I see daily the impact of that listening becoming real. Women report that they felt “seen” for the first time, that their partner became a better advocate, and that the birth felt like a team effort rather than a battle.

I hope that future conversations include doulas as part of the solution: not as luxury extras, but as key components of an equitable maternity care system where women’s voices and rights are central.

With respect and hope for meaningful change,

Here's a message from a Doula who shared her own journey of booking in with me:
Hi Julie

Thank you so much for the emails. I’m looking so forward to starting.

Bizarrely, I found you using ChatGPT. It’s amazing what AI can do!

I typed this in: ‘tell me some reputable South Australian birth doula training places or courses’, then I asked it to compare courses, then I asked it what one would be best for my circumstances, and you were the result! I started the search with mild curiosity which soon developed into I must do this course, it sounds amazing!! And within a couple of days, I’d signed up!! I guess it was just one of those ‘meant to be’ moments, it just felt right.

Your approach was much more in line with the type of learning/course I was looking for. It appears to have a much more personal, compassionate type method without the stress of what other courses have e.g. tests, essays etc. I have a nervous/anxious type of personality and didn’t want lots of pressure.

My key motivation at this stage is to become more knowledgeable about everything perinatal and the best ways to support those involved due to my becoming a grandparent in September/October. For many, many years I have had a passion for anything pregnancy related, and this gave me a great excuse to finally take the plunge with this sort of course. This is also instead of me doing endless googling, trying to discover lots of information, and doing my own research. Doing this course is giving me a much more structured, in-depth, well-rounded experience.
I am still completing the enrolment form, but will forward it to you soon.

I can’t wait to get started!

Take care

Busy Birth Doulas: Juggling Motherhood, Work, and Attending 10+ Births a Year

Being a birth doula is a calling. For many women, it’s not just a career — it’s a passion for supporting families at one of the most powerful and transformative times of their lives. But what about the birth doulas who are also mothers themselves, raising three (or more!) children while attending 10, 20, or even more births each year?

At first glance, it might seem impossible to balance family life, business, and the unpredictability of birth work. Yet many doulas do it beautifully, with organisation, support systems, and a commitment to self-care. Here’s how they manage.

Smart Systems and Practical Support

Batch Cooking & Meal Prep
Doulas with big families often rely on batch cooking — setting aside one afternoon each week to prepare multiple meals that can be frozen or reheated. Having a stocked freezer means children are fed and nourished even if mum is called away at 2 a.m.

An Au Pair in the Home
Many busy doulas take advantage of having an au pair — a live-in young person (often international) who provides childcare and light household help in exchange for food, lodging, and a modest allowance. An au pair can take children to school, keep routines stable, and ensure the household runs smoothly when mum is away at a birth.

A Supportive Partner & Neighbours
Behind every busy doula is often a supportive partner who shares responsibilities. Good communication and teamwork make it possible for the family to thrive. In addition, cultivating strong connections with neighbours can make all the difference — whether it’s a quick school pick-up, feeding the pets, or offering reassurance to the kids.

Professional Boundaries at Births

Experienced doulas with full lives set boundaries around when they arrive to support their clients. Instead of rushing over during early labour (which can last hours or even days), they wait until the labouring woman is in true labour — often confirmed by a hospital admission or a midwife’s exam showing the cervix is dilated to 4cm.

This ensures that doulas conserve energy, balance time away from their families, and arrive at the birth ready to give their full attention when it matters most.

Self-Care for the Busy Doula

Caring for others is impossible without caring for yourself first. Some favourite practices include:

Regular exercise (yoga, walking, or gym sessions that strengthen body and mind).

Mindful practices (short meditations, journaling, or breathwork between births).

Sleep hygiene (catching up on rest whenever possible).

Boundaries with clients (clear communication about availability and response times).

Emotional outlets (talking with fellow doulas, counselling, or journaling).

Emotional Intelligence & Daily Strategies

Balancing family and doula life isn’t just about logistics; it’s about emotional intelligence. Successful doulas lean on these skills:

Staying calm under pressure — modelling groundedness for both clients and family.

Practicing empathy at home — being as present with children as they are with clients.

Letting go of perfection — recognising that some days will be messy, and that’s okay.

Prioritising ruthlessly — focusing only on what matters most each day.

Thriving, Not Just Surviving

Busy doulas with multiple children prove that with creativity, support, and smart boundaries, it’s possible to attend more than 10 births a year and raise a thriving family. Their secret lies in systems, support networks, and remembering that self-care is not optional — it’s essential.

They show us all that balance doesn’t mean doing it all alone. It means creating a sustainable rhythm that allows both family and work to flourish.

✨ To every busy doula: you are living proof that it is possible to nurture your own family while helping others welcome new life. That is a profound kind of strength.
