Job Opportunity: Support Coordinator

Position: Support Coordinator
Location: Telarah, NSW
Hours: Flexible (15-30 hours per week)
Salary: Above award wages, based on experience

AffinitySC is seeking a dedicated and compassionate Support Coordinator to join our team in Telarah. This is an excellent opportunity for someone who is passionate about empowering individuals and making a meaningful difference in their lives. We offer flexible working arrangements, competitive pay, and a supportive team environment.

Key Responsibilities:
• Work with NDIS participants to develop and implement tailored support plans.
• Assist clients in navigating the NDIS and accessing appropriate services.
• Coordinate and connect participants with community resources, service providers, and funding opportunities.
• Monitor and review client progress, ensuring services align with their goals.
• Maintain accurate documentation and reporting in line with NDIS requirements.
• Advocate for clients to enhance their independence and quality of life.

Requirements:
• Experience in support coordination, case management, or a related field.
• Strong understanding of the NDIS and the disability sector.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and collaboratively.
• Strong organizational and time management skills.
• Proficiency in Microsoft Office and case management systems.
• A current driver’s license and reliable transport.
• Relevant qualifications in community services, social work, or a related field (desirable).

As we wrap up 2024, we want to take a moment to thank all of our amazing clients, informal and formal supports, and everyone who has been part of our journey this year. Your trust, collaboration, and support mean the world to us, and we couldn’t do what we do without you. 💛

This year has brought some exciting changes and new beginnings:

✨ Simone has taken a huge step and started her own support coordination service, Inspire Disability Network. We are so proud of her for following her passion, and we know she’ll make a tremendous impact. Joining her on this adventure is the wonderful Naomi, and we’re cheering them both on every step of the way!

✨ Denise has transitioned back into managing SIL houses, where she continues to shine. We know she’ll do amazing things (as always), and we’re thrilled to keep collaborating with her.
