Affinity SC is seeking an experienced Support Coordinator to join our growing team. This role is ideal for someone confident working with complex supports and passionate about delivering high-quality, person-centred coordination.

💼 Key responsibilities:
• Deliver NDIS Support Coordination (including complex cases)
• Advocate for participants and navigate complex systems
• Coordinate assessments, reports, and service delivery
• Build strong relationships with participants, families, and providers
• Maintain accurate documentation and compliance

✅ Essential criteria:
• Proven experience as a Support Coordinator
• Strong working knowledge of the NDIS
• Excellent communication and organisational skills
• Ability to work autonomously and within a team
• Current NDIS Worker Screening Check
• Driver’s licence and reliable vehicle

Hours: Flexible (15-30 hours per week)
Salary: Above award wages, based on experience

AffinitySC is seeking a dedicated and compassionate Support Coordinator to join our team in Telarah. This is an excellent opportunity for someone who is passionate about empowering individuals and making a meaningful difference in their lives. We offer flexible working arrangements, competitive pay, and a supportive team environment.

Key Responsibilities:
• Work with NDIS participants to develop and implement tailored support plans.
• Assist clients in navigating the NDIS and accessing appropriate services.
• Coordinate and connect participants with community resources, service providers, and funding opportunities.
• Monitor and review client progress, ensuring services align with their goals.
• Maintain accurate documentation and reporting in line with NDIS requirements.
• Advocate for clients to enhance their independence and quality of life.

Requirements:
• Experience in support coordination, case management, or a related field.
• Strong understanding of the NDIS and the disability sector.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and collaboratively.
• Strong organizational and time management skills.
• Proficiency in Microsoft Office and case management systems.
• A current driver’s license and reliable transport.
• Relevant qualifications in community services, social work, or a related field (desirable).

We are so proud of her for following her passion, and we know she’ll make a tremendous impact.
