Support Coordinator (Casual – Opportunity to Transition too Full-Time)

About the Role
Able Community Support Services is seeking a motivated Support Coordinator to join our growing team on a casual basis, with the opportunity to transition to full-time positions as the right candidate. You will support NDIS participants to understand and implement their plans, connect with appropriate services, and work towards achieving their goals.
Key Responsibilities
Support participants to understand and implement their NDIS plans in a person-centered manner
• Develop and maintain strong working relationships with participants, families, carers and stakeholders
• Link participants with appropriate disability, mainstream and community supports
• Assist participants to build capacity to coordinate their own supports where appropriate
• Monitor funding utilisation and ensure services remain within approved budgets
• Prepare progress reports and supporting documentation for NDIS plan reviews
• Advocate for participants to ensure their needs and goals are represented
• Identify and respond to risks, safeguarding concerns and barriers to service delivery
• Facilitate case meetings and collaborative planning with allied health and service providers
• Maintain accurate, timely and compliant case notes and documentation in line with NDIS Practice Standards
• Support participants during times of crisis or service breakdown
• Promote participant choice, control
About You
• Experience in Support Coordination or disability services
• Strong knowledge of the NDIS framework
• Excellent communication and organisational skills
• Ability to manage competing priorities
• Person-centred and strengths-based approach
Essential Requirements
• NDIS Worker Screening Clearance
• QLD Blue Card
• Current Driver’s Licence and reliable vehicle
• First Aid & CPR
• National Police Check
