Ready to step into a leadership role that truly makes an impact? 💙

We’re looking for a Team Leader – NDIS ILO to join our Cardiff team and guide our Support Workers and participants with confidence, compassion, and clarity.

About the Role:
As our Team Leader, you’ll oversee the day-to-day operations of our ILO service, ensuring supports are person-centred, empowering, and aligned with participant goals.

What You’ll Do:
• Manage a participant caseload (face-to-face)
• Lead, supervise, and mentor Support Workers
• Maintain compliance, quality standards, and KPIs
• Review documentation, reports, and incident records
• Run check-ins, meetings, and goal-planning sessions
• Build strong relationships with participants, families, and stakeholders
• Drive continuous improvement and support audits

In this role, Maryia helps guide new participants through a smooth and supportive intake experience, ensuring they feel informed, welcomed, and confident as they begin their journey with Sublime Care. 💙

Inspired by her mother’s work as a Registered Nurse and Community Nurse, Maryia is stepping into the disability and community care sector with heart, enthusiasm, and a genuine passion for helping others.

When she’s not guiding new clients through their onboarding journey, Maryia enjoys spending time with her nephew, playing video games with friends, diving into monthly Dungeons & Dragons sessions, exploring her newfound love of crocheting, and getting lost in a good book.

We’re looking for an Intake Coordinator to join our Wallsend team and help create smooth, supportive journeys for people accessing disability, aged care, and allied health services.

About the Role:
You’ll be the first point of contact for new clients — guiding them through their options, answering questions, and making sure they feel informed and supported from day one.

What You’ll Do:
• Support new referrals and help clients understand NDIS & aged care pathways
• Build relationships with referrers, providers, and community partners
• Keep records up to date and manage waitlists
• Work closely with our service delivery and clinical teams to ensure seamless care

Why You’ll Love Working With Us:
• SCHADS Level 3.1 – $38.65/hr + super
• Full-time Monday to Friday
• Work phone + laptop provided
