Would you debride this stable heel eschar?

Black tissue does not automatically mean “remove it”.

If the eschar is dry, firm and intact — with no erythema, fluctuance or drainage — it may be acting as the body’s natural biological cover.

The key question is not colour.
The key question is perfusion.

Debriding poorly perfused tissue can expose underlying structures and increase infection risk.

If perfusion is critically impaired, no dressing will fix the problem.

Lower limb wounds that are not healing should always trigger one question first:

Is the blood supply adequate?

🚨 Rest pain
🚨 Pallor on elevation
🚨 Dependent rubor
🚨 Non-healing ulcer with dry black eschar
🚨 ABPI < 0.5
🚨 Toe pressure < 30 mmHg
🚨 Sudden neurovascular change

These are not “review next week” findings.
They are urgent vascular red flags.

Compression without adequate arterial supply can cause harm.

Assess perfusion.
Escalate early.
Time is tissue.

Comvita Australia Pty Ltd is recalling certain batches of Comvita MediHoney Antibacterial Wound Gel due to problems with the packaging.

• Comvita Medihoney Antibacterial Wound Gel 25g
• Comvita Medihoney Antibacterial Wound Gel 50g

The packaging may not be sealed properly, which can make the product non-sterile or cause it to leak. Identified problems include an incompletely sealed foil liner and a folded tube seal that can leave a small gap.

If the packaging of a sterile product is damaged or inadequate, there may be a risk of product contamination and subsequent infection if the product is used on an open wound. It could also lead to delays in treatment if the product cannot be used as intended.

If you have purchased one of the affected batches of the product, do not use it.

🟤 IODOSORB™ (Cadexomer Iodine) – When & Why I Use It in Practice

One of the most effective topical antimicrobials we use in wound care for moderate to highly exudative wounds with bioburden is cadexomer iodine (IODOSORB™).

🔬 How it works:

• Absorbs exudate via hydrophilic starch beads
• Slowly releases iodine as fluid is absorbed
• Provides sustained broad-spectrum antimicrobial activity
• Helps disrupt biofilm and reduce odour

Unlike traditional iodine preparations, cadexomer iodine delivers controlled release, reducing cytotoxicity while maintaining antimicrobial effect.



📌 Indications:

✔ Venous leg ulcers
✔ Diabetic foot ulcers
✔ Pressure injuries
✔ Sloughy or infected wounds
✔ Critically colonised wounds with moderate exudate



🩹 How to use:

1️⃣ Cleanse and debride as indicated
2️⃣ Apply ~3mm layer to wound bed
3️⃣ Cover with appropriate secondary absorbent dressing
4️⃣ Change every 2–3 days or when paste turns grey/white

Reassess at 2 weeks as per antimicrobial stewardship principles.



🚫 Contraindications:

• Iodine sensitivity
• Active thyroid disease
• Neonates
• Caution in pregnancy, renal impairment, large wounds



As always, antimicrobial dressings should be used for clear clinical indication, aligned with Wounds Australia standards and antimicrobial stewardship principles.
