If you’re visiting YDHS in 2026 you might notice a bit of construction work happening.

We’re pleased to have been awarded funding as part of the Victorian Government’s Regional Health Infrastructure Fund to upgrade and enhance our aged care facilities at Crossley House and St Elmo’s.

As part of these upgrades, we’ll be adding five new beds to St Elmo’s to ensure we’re able to provide residential aged care services to locals from Yarram and surrounding communities and meet growing demand in the years to come.

We’re excited to have these works get underway and we know that the finished product will deliver wonderful benefits for our aged care residents now and into the future.

Support at Home (formerly known as Home Care Packages) is a Federal Government program that provides the support needed to help you live independently in your home for as long as you can.

The program provides a subsidy towards a package of care uniquely tailored to your needs.

Our skilled and experienced team of GPs provide a range of health and GP services and appointments including general medical appointments, script appointments, vaccinations, telehealth appointments, referrals and many more.

We also often have same day appointments.

With extreme heat conditions forecast across Victoria in the coming days, it’s important to look after yourself, your family and your neighbours.

Here are some simple steps to stay healthy and cool:

🌤 Stay informed
Keep up‑to‑date with heatwave alerts via the Bureau of Meteorology and local Victorian heat health warnings.

💧 Stay hydrated
Drink plenty of water, even if you’re not feeling thirsty.

🏠 Keep cool indoors
Use air‑conditioning or fans if you have them. Close blinds and curtains early to block out heat. Seek out cool public spaces like libraries or shopping centres if your home is too warm.

🚫 Limit heat exposure
Avoid being outdoors during the hottest part of the day. Postpone strenuous activities where possible.

🧒👵 Check on vulnerable people
Older adults, babies and young children, pregnant women and those with medical conditions are at higher risk during severe heat. Please check in on neighbours, friends and family.

🔥 Be fire ready
With increased temperatures comes elevated fire risk. Follow any local fire bans and stay aware of emergency updates.

If you start feeling unwell, especially dizzy, faint, or extremely thirsty, seek medical help immediately.

Stay cool. Stay safe. Look after each other.

There really have been so many highlights over the last year at YDHS.

In fact, there’s too many to name them all individually, but we thought we’d wrap up our look back over the last 12 months by giving a special shoutout to our volunteers.

As they do every year, our volunteers continue to offer so much value in a whole range of capacities across the health service.

Whether it’s transporting our aged care residents to outings and appointments, providing companionship, donating time and skills or fundraising, we’re lucky to have so many dedicated and selfless volunteers.

So to all of our volunteers, including our student volunteers, thank you for all your efforts throughout 2025.

We look forward to continuing to work together in 2026.

2025 was a big year for our Home Care Package clients and staff, with the transition to the new Support at Home program in November.

A huge thanks goes out to all our community care staff who worked tirelessly to ensure a seamless transition to the new Support at Home program.

And of course, we’d also like to thank our Support at Home clients for their patience and understanding during the transition.

We’re now two months into the new program and we’re looking forward to continuing to support our community care clients into 2026 and providing them with the supports they need to maintain their independence.
