What a day.. k-taping as a step down from splinting for bilateral DeQuervains, POSI splint for post OP ORIF of proximal phalanx and lastly a relative motion extension splint to increase flexion..

The infamous scaphoid fracture. It’s is the most commonly fractured carpal bone, accounting for 60-90% of carpal fractures. Men are usually young and the fracture is generally caused from a high energy fall on an outstretched hand (FOOSH). Women are usually elderly and the cause is a fall from standing. The scaphoid is sneaky and sometimes likes to play a game of hide and seek on an initial X-ray. As a therapist if we get someone in our office with a suspected scaphoid fracture with all clinical assessments indicating a scaphoid fracture but a clear X-ray we would splint the client and treat them as if they had a scaphoid fracture and request follow up scans. I will finish my small novel off with two indications of a scaphoid fracture 1) wrist pain and 2) anatomical s***f box pain.

Extensor Tenosynovitis - DeQuervains Tendinopathy.
What is it:
-a painful condition that effects the extensor tendons where they run through a tunnel on the thumb side of the wrist
-stenosis of the first extensor compartment caused by thickening of the sheath encasing the tendons which then can lead to inflammation of the tendon/s

Epidemiology:
-more common in women
-general population age is usually 40+ years
-post partum average age 33 years

Causes:
-overuse of the thumb and wrist - repetitive use due to an occupational task or hobbie
-can be associated with pregnancy and post partum due to postures adopted when breast feeding
-people with rheumatoid arthritis are susceptible to developing DeQuervains tenosynovitis

Treatment:
Research shows that if caught in early stages that DeQuervains tenosynovitis responds well conservative management with hand therapy

As a therapist I believe that a different therapy must be constructed for each patient because each patient has a unique story.
