PARDWANA TECHNIQUE DURING CESAREAN SECTION in deeply arrested cephalic presentation
Delivery of deeply impacted baby in the pelvis incephalic presentation by (Pardwana method).In the above-mentioned case head was engaged such arrested in the mid cavity that catheterization was difficult.after reengaging baby's head catheterization was done. Then taking all asepting measure Cesarean section done with a little bit high incision than usual level & incision extended further. Initially baby's upper arm is released.Then front lower extremity gripped followed by another lower extremity gripped, eventually gripping pelvis of baby body is delivered folled by trunk & head delivered easily. In deeply impacted head is advised not trying pushing head through introitus causing injury to birth canal or to Bay or inviting infection to mom.

Twin pregnancy- Superfetation a rare clinical entity.
In our daily obstetric practice twin pregnancy is not an uncommon occurence. A case 25 yrs. history of oid, early sonographic sono study reaveled single intrauterine pregnancy subsequently mom develop excessive vomiting. After 4 weeks interval 2nd songraphic report revealed pregnancy with cardiac actvity & another pregnancy but no cardiac activity of 2nd twin. A 3rd scan shows both tween having cardic actviy with seperate placentae with defferent biometric measurements of both fetuses. And diagnosed as superfetation, when two infants are grown from two ova from subsequent cycle. Where as if two ova are fertilized by seperate o**m in same cycle are termed superfecundation & in later case both the babies are of same biometric messurements and same placentatal maturity . In the above mentioned case of superfetation twin pregnancy there is chance of iatrogenic prematurity if later baby tween who conceived in next cycle, delivery taken place before fullterm. So avoiding iatrogenic prematurity, strong suspicion is necessary in case early dating sonographic finding of singleton pregnancy & subsequent sonographic twin pregnancy reporting with exaggerated pregnancy symptom & sign. Subsequent discordent growth scan & maturity study may be guide to this rare clinical entity.

Mrs TANJINA AKTER( name changed) Aged 35 child less for 15 years. History of 5 RPL, develop diabetes after last conception 4 year back .She came to me for subfertility .Her diabetes controlled with oral medicine metformin 500 mg daily. APA test found negative .her endometrial thickness was found reduced around ovulation. Naltrexone + sildenafil was used continuously for 6 month , and she conceived & continue her ANC accordingly without much hazard. Her diabetes controlled with diet & oral medicine metformin only. Electively she was delivered through cesarean section after completion of 38 weeks with a healthy baby without any complication for mother and baby.

A case history .
9 yrs. of secondary subfertility ē on miscarriage 4 yrs. ago, histoy injudicious indigenous d&c back Same came to me . sonography revealed urcuate uterus ē balateral pcos. Treated accordingly and conceved after 9 month of conservative treatment. Found two fetuses after 8 week. there after 16 week itwas revealed monozygotic tween confermed by sonography. But after passing another 4 week there was found one fetus not alive. Pregnancy continue with close monitoring upto 37 cmpleted
week. Of both baby one alive A/S 8/10 & other dead baby boy were delevered through LSCS on 09 10 23 No neurological decicit was elicited. Both baby & mom are healthy now. Thanks GOD.

Uterine niche is a late complication of cesarean deliveries and causes chronic pelvic pain, menorrhagia or postmenstrual spotting, and infertility. As the number of cesarean sections are constantly increasing, it is important to be aware of this entity so as to make an early diagnosis. This would enable the clinicians to manage these patients efficiently.
