Thyroid Imaging Reporting and Data System (TI-RADS)
TI-RADS is a standardized ultrasound-based classification system used to assess thyroid nodules and estimate their risk of malignancy, helping guide the need for fine-needle aspiration (FNA) or follow-up.
Developed by: American College of Radiology (ACR)
Ultrasound Features Assessed
Each nodule is scored based on:
1. Composition (cystic, solid, mixed)
2. Echogenicity (anechoic to very hypoechoic)
3. Shape (wider-than-tall vs taller-than-wide)
4. Margins (smooth, irregular, extrathyroidal)
5. Echogenic foci (calcifications)
TI-RADS Categories
TR1 – Benign (0 points)
TR2 – Not suspicious
TR3 – Mildly suspicious
TR4 – Moderately suspicious
TR5 – Highly suspicious
(General Guidance)
TR1–TR2: No FNA
TR3: FNA if ≥2.5 cm
TR4: FNA if ≥1.5 cm
TR5: FNA if ≥1.0 cm
-Reduces unnecessary biopsies
-Improves communication between radiologists and clinicians
-Standardizes reporting and management decisions
