আমার এক কলিগ কাম ছাত্রের সকালের প্রশ্ন :

আসসালামু অলাইকুম সালাম স্যার

আমরা ইনডাকশন এর সময় যে ফেন্টানিল দিই তা ২০-২৫ মিনিট এ শেষ হয়ে যায়।

এরপর এনালজেসিক হিসাবে নাইট্রাস ইউজ করি।
নাইট্রাস এর MAC -104.

আমরা ৫০-৬৬% নাইট্রাস ব্যাবহার করি তাতে ০.৪৮-.৬ ম্যাক অর্জন হয়। আইসো সাথে দেবার জন্যে আইসোর ম্যাক কিছুটা কমে ১.২ থেকে ০.৬ হয়। নাইট্রাস এর ম্যাক ও কিছু টা বাড়ে।

এইটুক ম্যাক দিয়ে কি স্যার ওটি চলাকালীন প্যাশেন্ট এর পুরো এনালজেসিক কাভারেজ হয়? নাকি আরো দেওয়া উচিত লম্বা ওটি গুলোতে?
প্রথম প্রশ্নের উত্তর :

Q❤️.1.whether N2O is analgesic in 33% MAC value?
Ans:yes.
Analagesia start from 15/20% to 40% MAC

Q❤️.2.If yes,is it sufficient?
Ans: No. not sufficient
So we can use opioids

Q❤️.3.No OT with nitrous oxide?
A.
We can
1.Dressing
2.Labour analgesia
3.minor orthopedic surgery

Q.4❤️.why we use nitrous oxide though it has 104%
MAC?
Or
Did we need to achieve 104% for analgesia??

❤️Ans:
We don't use N₂O alone for general anesthesia because its MAC is >100%, but we use it as a supplement because:

It reduces the required dose of other anesthetics

It provides analgesia

It speeds up induction and recovery

It's safe and well tolerated in many patients

so, 2 ampoule inj KT in 0.9% NaCl can cause
Increase 0.5-1 mmol/L
Not in peeipheral vein less than 4 hour in ward(10 mmol/h)
In Central line less than 20mmol/h
