Breast Density…

In women with the densest breasts there is a 4x increased risk compared with women with fatty breasts.

Almost 10% of women in the 50–70-year age group have extremely dense breasts. Women with dense breasts have an increased probability of their cancer being detected at a later stage at screening, or as an interval cancer (cancer detected between screening rounds), with resulting worse prognosis.

Ultimate Imaging have provided a breast density report for our patients since 2018. For those women with dense breasts, supplemental imaging may be required.

Automated breast ultrasound (ABUS) has proven to be a viable supplemental screening tool for dense breasts, offering wide accessibility, no contrast or radiation, and better tolerability for women.

ABUS is provided as a supplementary screening tool here at Ultimate Imaging.

Is there LIGHT at the end of the tunnel?

Ongoing studies are looking at ways to enhance current breast cancer screening: options:

– Several studies are investigating the use of mammography with a contrast dye to improve screening among women with a history of breast cancer and women with dense breasts.

– Some researchers are exploring the use of markers such as breast stiffness to identify cancer in women with dense breasts.

– Researchers are looking for ways to use artificial intelligence (AI) to improve early detection of breast cancer.

The work continues to advance our understanding of how to prevent, detect, and treat breast cancer, addressing disparities and improve quality of life for survivors of the disease.

A difficult dilemma – Men and breast cancer

It is important that men realize that they too can develop breast cancer. Equally important is that the taboo surrounding male breast cancer gets lifted. Men who are affected by the disease experience great difficulty in talking about their illness.

It is a rare cancer in men, with only 0.3% of all cancer diagnoses annually. In general, the disease is diagnosed at a later age due to the fact that there is no general screening program for men. Because of this late detection, the disease tends to be more advanced in men at the moment of diagnosis. As a result, outcomes are slightly worse than they are for women. The 10-year survival in men is around 5% lower than in women.

Treatment of male breast cancer is not quite the same as in women. Chemotherapy and radioation are applied to both sexes, but with men complete breast removal is often the norm. Men also receive more hormone therapy than women: 83% compared to 55%.

Recently, breast-saving operations have started to be performed with men, and to a degree breast reconstructions are also possible.

Is breast asymmetry linked to breast cancer?

Breast asymmetry, or uneven breasts, refers to when one breast is a different size or shape than the other. Many females have this, and it is not typically a cause for concern. However, substantial asymmetrical breast size or density may be a sign of a higher risk of breast cancer.

In fact, those with dense breasts are at risk of developing breast cancer four to six times higher compared with women of the same age with low breast density.

Any unusual changes in the breast should undergo an examination by a doctor and annual mammograms can test for abnormalities or changes in breast tissue.�

Bermuda’s healthcare spending per capita is one of the highest in the world, up to 35% of the population remains unable to access quality healthcare without financial hardship. This includes those who are uninsured (about 12%); or have defined unaffordable health insurance or are underinsured (about 23%).

This economic burden must be addressed as well as ensuring our community has availability of primary services and treatments.

Breast density after menopause.

Breast density may decrease after menopause in both women who go through natural menopause and younger women who are in menopause after surgery to remove the ovaries (oophorectomy).

For women who use hormone therapy (MHT/HRT), breast density may not decrease until they stop using MHT/HRT.

Please be sure to inform your radiologist if you are taking any of these hormone therapies.
