Reporting to the Casa Health/HealthLink Outreach Managing Director, Casa Health/HLO leadership designate, and nurse leadership/team member(s). The C-RCA operates under the functional direction of Casa Health Licensed Practical Nurses (LPN), or other designates, as assigned. The Community Registered Care Aide (C-RCA) acts as an integral member of the Casa Health/HealthLink Outreach care team Chilliwack, Abbotsford and Mission areas; and assisting in delivering patient care while maintaining a person-centred approach to all care provided in each patient’s home.
Casa Health/HLO services are based on medication and chronic disease management care models. Thus, the C-RCA will be responsible for working closely with nurse leadership and pharmacy to ensure safe and complete care is delivered per patient care plan(s).
The Casa Health/HLO C-RCA will use their passion for healthcare and their developed skill(s) to improve Casa Health/HLO patient’s lives. They will act as an integral member of the patient(s) care team to support each individual to maintain their independence, dignity and holistic well-being.
Key Responsibilities
Following established care plan(s), the Casa Health/HLO C-RCA provides care services to patients, including, but not limited to:
Administering medications to patients per scheduled assignment/route list(s), provided by Casa Health/HLO, as training/certification permits, only.
Working in close collaboration with the Casa Health/HLO nursing and pharmacy team(s) to provide safe, competent, and appropriate support to chronically ill patients.
Completing basic vital signs monitoring and tracking, utilizing appropriate automated equipment and assigned documentation.
Reporting emergencies and care issues per the “What, When, How” sheet to nurse leadership and/or pharmacy, immediately.
Completing Casa Health/HLO medication and C-RCA training and education per schedule. Participates in ongoing training (annual education calendar, etc.)
Striving to build rapport with and support/encourage each individual patient to be at ease with self/team, thereby allowing for more effective care partnerships to be developed/maintained.
Observing and monitoring client/resident progress, symptoms and behavioural changes and reporting/documenting significant observations in each patient’s “C-RCA PROGRESS NOTES.”
Maintaining a clean and safe environment for clients and staff.
Maintaining equipment and supplies per “C-RCA Bag Sheet”.
Performing other related duties per delegated or assigned tasks ONLY.

Follows standard precautions to prevent the spread of infection between caregiver and client/resident. All required PPE is utilized at all times.
Advocates for each patient when concerns arise related to well-being/safety.
Develops and maintains effective therapeutic relationships with patients and their family members.
Recognizes potential conflict of interest opportunities and report to nurse leadership.
Promotes Casa Health/HLO verbally and with quality of service; does not solicit business from any Casa Health/HLO patients independent of Casa Health/HLO.
Exhibits a commitment to providing high-quality healthcare services with a person-centred approach.
Shows dedication to effective teamwork as well as maintaining optimal communication with leaders, peers and patients.
Commitment to continuous education.
Exhibits knowledge and the daily practice of the Casa Health/HLO model of care and policy & procedures.
Participate in ongoing educational opportunities such as in-services and other education/training provided by Casa Health/HLO, per education calendar and as scheduled.
Works closely with nurse leadership, pharmacy team and nursing team to ensure safety for C-RCA and patient(s) at all times.
Key Relationships
Casa Health care team members (C-RCA’s) report to the Casa Health/HLO Managing Director and Casa Health/HLO Leadership designate(s), under the functional direction of Casa Health/HLO, Licensed Practical Nurses (LPN’s), or other designates(s).
The Community Registered Care Aide (C-RCA) is an integral part of the care team and assists the Casa Health/HLO care team(s) in delivering patient care services and works with the direction of:
Casa Health/HLO leadership.
C-RCA team members.
LPN’s (Casa Health and HealthLink Outreach).
Human Resources (as required).
Qualifications and Competencies and Requirements
Graduation from a recognized Resident Care Aide (RCA) program.
Current registration with the BC Care Aide Registry is required.
