The Biology of Rickets:

Rickets develops when the body lacks enough vitamin D, calcium, or phosphate to build strong bone. Vitamin D is especially important because it allows the intestines to absorb calcium from food. Without it, calcium levels fall and the body cannot properly harden the collagen framework that forms new bone. Instead of becoming rigid mineralized tissue, the bones remain soft and flexible.

In children, bones are still growing from regions called growth plates. These plates are made of cartilage that normally becomes mineralized bone as a child develops. In rickets, that mineralization step fails. The cartilage thickens, the bone underneath weakens, and the skeleton becomes vulnerable to bending under everyday forces like standing, walking, and gravity.

That is why rickets often produces distinctive skeletal changes. The legs may bow outward, the rib cage can develop bead-like enlargements where ribs meet cartilage, and the pelvis may become distorted. In severe cases, the spine curves dramatically, producing scoliosis or a double “S-shaped” curvature like the one seen in this specimen. Over time, the softened vertebrae and long bones deform permanently.

Today, rickets is far less common in industrialized countries thanks to vitamin D supplementation and fortified foods, such as milk and cereals. However, it has not disappeared.
