The Crowsnest Medical Clinic will be closed on Monday, October 13th for Thanksgiving Day.

In collaboration with the Chinook Primary Care Network, we are asking patients to complete a Patient Experience Survey after each appointment. Patients will receive a notification via text or email for a Qnaire from our EMR that will redirect them to a short online survey. Paper copies and a poster with a QR code linked to the survey are also available in clinic. This will run until the end of November. We appreciate everyone taking the time to let us know how we're doing! Thank you!
