ConsidraCare is proud to announce the first of many pharmacy-integrated care pilots across the Greater Toronto Area.
We’re starting with Markland Wood Pharmacy in Etobicoke, and building a scalable model that connects trusted community pharmacies with nurse-managed home care.
Why pharmacies?
In a recent Ipsos poll for the Ontario Medical Association, 85% of Ontarians reported a favorable impression of pharmacists. That’s tied with registered nurses (85%) and ahead of doctors (80%).
90% of Ontarions live within 5 km of a pharmacy.
Pharmacies are not just accessible. They are trusted. Imagine if they can become local community hubs for senior care.
When families are navigating recovery or managing complex needs, the pharmacy is often the first place they turn. This pilot is about making sure the support doesn’t stop at the counter. We’re doing that by equipping pharmacy teams with home care guidance and placing simple in-pharmacy info stations that connect families to nurse-led support.
