We want your help!

We are creating a tool to support people after they leave the hospital, especially if they came to the Emergency Department for mental health or substance use concerns.

What is this tool?
It’s a simple sheet of paper that patients get when they go home. It explains:

What to do next
Follow-up appointments
How to take medications safely
Community supports and services that can help

Why your voice matters:
We want to hear from people with lived experience—whether you have personally experienced mental health or substance use challenges, or have supported someone who has. Your feedback helps us understand what would make this tool easy to use, clear, and truly helpful.

Your ideas will help shape the final version of the tool, which will be tested in a hospital setting.

Nurse Practitioners support patients and families every day, providing care, guidance, and compassion in clinics, hospitals, long-term care, mental health services, and in the community. Their work helps make care more connected and accessible close to home.

Today, we remember.

We honour those who served and sacrificed so we could live with the freedoms we have today. We hold in our hearts the families who were never the same, the loved ones who waited and grieved, and the communities forever changed.

We also remember those who returned home carrying wounds seen and unseen, as well as the medical teams, nurses, doctors, and caregivers who supported them then and continue to support veterans today. Their care, compassion, and dedication are part of this legacy too.

To all who served, to those who continue to serve, to the families who have given so much, and to the caregivers who help heal and guide — thank you.

You’re invited to join us for a Trans Day of Remembrance event hosted by Canadian Mental Health Association, Brant Haldimand Norfolk Branch Brant Haldimand Norfolk and the Brantford Brant Norfolk Ontario Health Team.

The event will feature Mabe Kyle, CMHA staff member and editor of Q***r Country Crossroads, who will share a short reading and lead a conversation on remembrance, belonging, and the connections between systemic discrimination and mental health.
