Need help with mobility? We can help!

Revive Occupational Therapy is an authorized provider of ADP (Assistive Devices Program) assessments. Through the ADP, eligible Ontario residents can receive funding assistance (75%) for mobility devices such as:
• Walkers (rollators)
• Manual wheelchairs
• Power wheelchairs
• Mobility scooters

We offer fast assessment appointments with reports completed within 1 to 2 weeks!

To qualify, individuals must have a physical impairment that impacts mobility for longer than six months. If you or a loved one is struggling with mobility and think you may be eligible, contact us today to schedule your assessment.
