Health Quality Alberta Improving Healthcare Together We are grateful to work, live and learn on the traditional Treaty territories and Métis lands in Alberta.

Health Quality Alberta is a provincial agency that brings together patients, families, and our partners from across healthcare and academia to inspire improvement in patient safety, person-centred care, and health service quality. We assess and study the healthcare system, identify effective practices, and engage with Albertans to gather information about their experiences. We value diverse perspectives, and bring objective, evidence-based analysis to our work. Our responsibilities are outlined in the Health Quality Council of Alberta Act.

Patient advisors with lived experience worked with staff to design, test, and implement the guide across acute care sites in Alberta. Hear how My Next Steps is helping to improve transitions in care to reduce the risk of readmission or return visits to emergency.

We were proud to recognize the work of patient advisors and staff in the design, testing, and implementation of this resource with a 2025 Patient Experience Award.
