A reminder of why accessible pharmacy care matters đ
When a patient noticed her blood pressure readings were staying high at home, she stopped by the pharmacy for guidance. Her physician was away, and even though she was taking her medication as prescribed, something clearly wasnât right.
After assessing the patient by conducting a medication review, examining readings and medical history, the pharmacist adjusted the dose of her blood pressure meds. To support continuity of care, the interaction was documented, and her doctor was notified of the change. The pharmacist arranged followâup blood pressure checks, and over the next few weeks, her numbers returned to target and remained stable. When her physician returned, they confirmed the results and renewed the updated dose.
Sometimes, the right care is right around the corner.
