ConsortiaCare.ca provides enhanced home care services led by Registered Nurses (RN's) with added training in supportive care, counselling, nutrition & exercise coaching. When patients are referred and accepted into one of our home care programs, we provide an added layer of home supports to address problems before they happen and manage complex disease conditions at home. Our programs are more comprehensive than other options in the community. But, we specifically aim to improve health-related Quality of Life (HR-QoL), measuring what activities people have already given up alongside how their disease progressing.
