For help finding financial support, ask your primary care provider to refer you to a PCN social worker. Social workers are experts in navigating community resources - they can talk to you about services you may qualify for or that you may not know about, including:

✅ Community financial services
✅ Disability services
✅ Medication coverage
✅ Employment counselling
