Before you chase the psoas… try looking here.⬇️

When someone shows up with back pain, most people immediately think psoas, iliacus, or QL. The posterior abdominal wall gets all the attention.

But here’s the bigger question:
Why don’t we look at the anterior abdominal muscles with the same level of suspicion?

Your re**us abdominis, obliques, and TVA play just as big a role in spinal mechanics as the deep posterior muscles. When they’re weak, inhibited, scarred, or overprotective, the back ends up doing all the work—and getting irritated for it.

Re**us Abdominis

If rec abs aren’t contributing, the spine loses its flexion counterbalance → paraspinals overwork → the psoas guards.

Obliques

Underactive obliques shift rotation into the lumbar spine → facet compression → QL tightens.

TVA

When TVA isn’t regulating pressure, the spine loses segmental stability → psoas and multifidus over-recruit to compensate.

Bottom Line

Back pain isn’t just a posterior issue.
Sometimes the problem is up front.

So when someone says, “My back hurts,” don’t stop at the psoas.
Check the anterior abs.
They might be the missing piece.

How is RAPID so different from what you are already doing?

A great picture and study that needs to be learned and shared. A great deal of this is exactly what we talk about in RAPID.

People often wonder why there is a propensity for acute pain to turn to chronic pain.

Unfortunately the same folks rush to stop inflammation every chance they get, not realizing that the pro inflammatory stage is an essential part of healing.

Another little known fact is that it must complete fully to move to the next stage. Stopping inflammation retards this process and some injuries become stuck and progress to chronicity.

Notice on the diagram the M1 and M2 macrophages. In the early pro inflammatory phase machrophages are polarized to an M1 presentation, this is valuable as it cleans house and protects the tissue from infection. Over time as the pro inflammatory phase completes machrophages polarize to M2. Theses macrophages become anti inflammatory and move the body to tissue repair.

Again as these phases naturally progress where the pro inflammatory phase is able to complete, this progression from M1 to M2 happens without effort.

Unfortunately due to the common halting of the pro inflammatory phase, this natural progression halts and the macrophages remain at M1. This becomes a barrage of inflammation and tissue destruction. This is the reason that osteoarthritic joints become broken down and painfully inflamed.

With RAPID we appreciate this natural cycle and reignite the pro inflammatory phase, with it also comes the release of substance P.

By restarting the healing cycle, we flush out the old aberrant inflammation and the inflammatory irritants. Substance P then comes in and repolarizes the macrophages from M1 to M2. This now begins the anti inflammatory phase and moves the tissue toward repair and healing.
