November 16-22 is Digital Health Week - and to close out the week, we're shining the spotlight on our Quality Improvement (QI) Team. They're the small-but-mighty group behind safer, smoother, more patient-centred care and operations.

Every day, this team leans into complexity so that care can feel simple for patients and providers. They:
📈 Turn data into better care: analyzing EMR and practice-level data to understand demand, capacity, and access, and helping teams redesign schedules and workflows.
🙌 Support day-to-day operations: co-designing data dashboards, streamlining activity reporting, and supporting or leading projects and initiatives across the organization, at HUB sites, and within practices.
✅ Improve patient experience: refreshing patient experience surveys, tracking feedback, and partnering with Patient Experience Advisors so real voices drive real change.
💪 Strengthen equity and access: using data and community insights to focus on patients who face barriers to care and supporting initiatives like attachment and social medicine hubs.
💻 Advance digital tools: helping teams get the most out of online booking, eReferral, and other digital tools so clinicians spend more time with patients and less time wrestling with systems.

Today is World Diabetes Day!

Did you know we offer free Diabetes Prevention and Diabetes Management workshops that are open to the public? These workshops are led by a Registered Dietitian and a Pharmacist - learn more and register online: https://buff.ly/KwUeqAG

Your primary care team including your Family Doctor, Registered Dietitian and Pharmacist work together to support you in your diabetes management. Last week, members of our Hamilton Family Health Team leadership team participated in the KAIROS Blanket Exercise, developed by .canada and facilitated by the incredible . We were warmly welcomed into De dwa da dehs nye>s (), who graciously hosted us in their space for this exercise.

Afterwards, the team described the way our hearts, minds and eyes were opened to the truth of colonization on Turtle Island, the ongoing ways that Indigenous communities are still targeted by systemic oppression, and how we can participate in reconcili-action as we move forward in our work and lives.

We are incredibly grateful to Teresa and Sarah from for creating such a powerful and healing space for us. It is an experience that will stay with each of us and ripple outwards into how we care for each other and our community. We would also like to extend our sincere thanks to Janet Gasparelli and the team at De dwa da dehs nye>s for hosting us in their space and for their continued partnership as we serve our community together.

It's National Nurse Practitioner Week! 🎉

Join us in celebrating Nurse Practitioners (NPs), at the Hamilton Family Health Team and across Canada, who make a difference every day with compassionate, expert care. As primary and specialty care leaders, NPs are essential in creating a healthier, more accessible healthcare system for everyone.

NPs are highly educated healthcare professionals with foundational training and education in nursing. NPs play a unique and complementary role in the healthcare system, practicing autonomously while integrating advanced clinical knowledge, diagnostic skills, and specialized expertise. Rooted in whole-person and patient-centered care, NPs consider the social determinants of health to provide comprehensive and individualized patient care.

Today, we remember all of Canada's veterans, honouring their courage, selflessness and sacrifice.

We will all be taking two minutes of silence to remember and reflect on the sacrifice of all our veterans, including racialized soldiers who overcame the additional barriers of discrimination and often went unrecognized, and the more than 12,000 Indigenous peoples who served our country for more than 200 years despite not being afforded basic human rights and being subject to the conditions of colonial governance.

Did you know we offer two free health education groups for the 2SLGBTQIA+ community? The Hamilton Family Health team recognizes, along with major health care organizations in Canada and around the world, that gender affirming care is healthcare and that all members of Hamilton's 2SLGBTQIA+ community deserve to feel safe, respected, and heard when accessing care.

The Gender-Affirming Care Workshops for adults (19+), in collaboration with Queens Square Family Health Team, offer participants evidenced-based information from healthcare providers on both masculinizing and feminizing Hormone Replacement Therapy (HRT).

Our eight-session AFFIRM group, in collaboration with Compass and the Hamilton-Wentworth District School Board, provides 2SLGBTQIA+ youth ages 11-18 with a supportive space to explore identity, manage emotions, and build confidence. Sign up for our waitlist to be notified once new dates for AFFIRM are announced.

Last Saturday, the , the and collaborated to host a Community BBQ and Wellness Fair for residents of the L8L postal code! 🍔

Does your child need vaccines or immunizations? Come to our free vaccine clinic at Eva Rothwell Centre: Offering infant/childhood and school aged vaccines (HPV, Hepatitis B, Meningitis, Measles, Tetanus, Polio, Chicken Pox, whooping cough and RSV vaccine).

Drop in any time this Saturday, October 4 between 11:00 am to 2:00 pm at Eva Rothwell Centre (460 Wentworth Street N., Pethers Community Room). No appointment necessary and no health card needed but please bring yours if you have one! Snacks will be provided.

October 1 is National Seniors' Day!

We believe that healthy aging isn’t just a goal for October, but a lifelong journey worth celebrating every day. That’s why we're proud to offer a free Healthy Aging Workshop Series, designed for seniors and their caregivers. These are free, one-hour workshops that are held in-person and virtually.

Each session is facilitated by healthcare professionals such as registered dietitians, social workers, pharmacists and physiotherapists, who understand the unique needs of older adults and offer compassionate, expert guidance.

Come walk with us and learn some tips for mindful living! 🚶

Join us at Bayfront Park tomorrow, Friday July 25, from 9:00 - 10:00 a.m. for another Just Walk event, powered by : a free, one-hour walking program designed to bring the community together while promoting better health.

Tomorrow's session, hosted by Tricia Brinn, MSW, RSW and Mental Health Groups Coordinator, will be all about mindful ways to manage stress. Check out this tip from Tricia and we hope to see you there tomorrow!

"Wellness doesn’t just have to be hour-long meditations or complex routines. Consider weaving micro-practices into your day—brief, intentional pauses that can support your well-being over time. These might include a few mindful moments while walking, sipping your coffee, eating, showering, doodling, or even doing dishes. Small shifts in everyday habits can create powerful changes in how we feel."
