March is Pharmacy Appreciation Month: a time to recognize the impact clinical pharmacists make across the healthcare system every single day. This month we celebrate the 18 clinical pharmacists who are integral members of Hamilton FHTs interprofessional teams.

This month, we thank you for your expertise, compassion, leadership, and commitment to safer and more effective medication use. Clinical pharmacists in primary care are not just medication managers; They are educators, collaborators, patient advocates, and system improvers.

Meet Cait Luther, a clinical pharmacist and Menopause Society Certified Practitioner (MSCP): "I was drawn to clinical pharmacy because it combines my interest in pharmacology with the opportunity to directly improve patient outcomes."

Hamilton Family Health Team is proud to celebrate Dietitians Day (March 18, 2026)

Our Registered Dietitians (RDs) play an essential role in helping patients and families build healthier relationships with food that improve overall health and well-being.

Meet Diana Al-Farraji, a Registered Dietitian with the HFHT: "Coming from a family that enjoys food, home-cooked meals and listening to nutrition talks on TV, piqued my interest in learning more about food, nutrition science and the important role it plays in our day-to-day lives." Diana continues to share, as a health care provider; she translates nutrition science into recommendations, tips and strategies to help people meet their health goals. In the winter, you can find Diana skiing.

We are grateful for the knowledge, compassion, and dedication our dietitians bring to our team and to the Hamilton community every day. Thank you, RDs, for the important work you do to support healthier lives.

March 2026 is a time to recognize the essential contributions of social workers across Ontario and Canada. Ontario Social Work Week (March 2–8) and National Social Work Month (March 2026) highlight the profession’s deep commitment to supporting individuals, families, and communities through compassionate, evidence‑informed care.

Meet Mona Huang, a registered social worker: "I was drawn to social work because of its commitment to advancing social justice and equity. Social work recognizes the interconnections between individual challenges and broader systemic inequities, and works across structural, community, and individual levels to create meaningful social change." You can find Mona enjoying rock climbing when she is not at work.

National Non‑Smoking Week (January 18 to 24, 2026) is a powerful reminder that choosing to quit, or even cutting back, is one of the most meaningful steps you can take for your health. Everyone’s journey looks different, and you don’t have to navigate it alone. We’re here to support you in finding the path that fits best for you.

Two ways we can help:

1. Our Quit Smoking Workshops are free and open to the public. Join a supportive community, learn about different options and access 26 weeks of free ni****ne replacement, led by a pharmacist and nurse. To register or learn more, call (905) 667-4852.

Free Upcoming Monthly In-Person Quit Smoking Workshops:
➡️ Thursday, February 19, 12:00 pm - 1:00 pm (688 Queensdale Ave. E., Suite 3, Hamilton): Building New Habits, Part 2 - We continue our discussion on habits to include ways to shift your focus towards becoming a non-smoker.
➡️ Thursday, March 19, 12:00 pm - 1:00 pm (123 James Street North, Suite 200 Hamilton): Smoking cessation and mood management.

How does it work?

The National Day of Persons with Disabilities is observed on December 3, 2025, and is dedicated to promoting awareness, understanding, and acceptance of people with disabilities. The theme for 2025 is "fostering disability inclusive societies for advancing social progress". Adults with disabilities too often encounter barriers to healthcare and full participation in their communities, limiting their independence, opportunities, and wellbeing. The Hamilton Family Health Team believes that when barriers are removed people with disabilities are empowered to participate fully, and communities thrive.

Today is National Physician Assistant (PA) Day! We're highlighting all of the incredible Physician Assistants who serve our Hamilton community and beyond. 🎉

PAs are highly skilled medical professionals who collaborate with physicians to diagnose, treat, prescribe, and assist in surgery, enhancing care across every setting. PAs have a positive impact on many aspects of health care, from emergency room wait times, to access to primary care, the quality of seniors care, and access to health care in rural communities.

November 16-22 is Digital Health Week - and to close out the week, we're shining the spotlight on our Quality Improvement (QI) Team. They're the small-but-mighty group behind safer, smoother, more patient-centred care and operations.

Every day, this team leans into complexity so that care can feel simple for patients and providers. They:
📈 Turn data into better care: analyzing EMR and practice-level data to understand demand, capacity, and access, and helping teams redesign schedules and workflows.
🙌 Support day-to-day operations: co-designing data dashboards, streamlining activity reporting, and supporting or leading projects and initiatives across the organization, at HUB sites, and within practices.
✅ Improve patient experience: refreshing patient experience surveys, tracking feedback, and partnering with Patient Experience Advisors so real voices drive real change.
💪 Strengthen equity and access: using data and community insights to focus on patients who face barriers to care and supporting initiatives like attachment and social medicine hubs.
💻 Advance digital tools: helping teams get the most out of online booking, eReferral, and other digital tools so clinicians spend more time with patients and less time wrestling with systems.

Last week, members of our Hamilton Family Health Team leadership team participated in the KAIROS Blanket Exercise, developed by .canada and facilitated by the incredible . We were warmly welcomed into De dwa da dehs nye>s (), who graciously hosted us in their space for this exercise.

Afterwards, the team described the way our hearts, minds and eyes were opened to the truth of colonization on Turtle Island, the ongoing ways that Indigenous communities are still targeted by systemic oppression, and how we can participate in reconcili-action as we move forward in our work and lives.

We are incredibly grateful to Teresa and Sarah from for creating such a powerful and healing space for us. It is an experience that will stay with each of us and ripple outwards into how we care for each other and our community. We would also like to extend our sincere thanks to Janet Gasparelli and the team at De dwa da dehs nye>s for hosting us in their space and for their continued partnership as we serve our community together.

It's National Nurse Practitioner Week! 🎉

Join us in celebrating Nurse Practitioners (NPs), at the Hamilton Family Health Team and across Canada, who make a difference every day with compassionate, expert care. As primary and specialty care leaders, NPs are essential in creating a healthier, more accessible healthcare system for everyone.

NPs are highly educated healthcare professionals with foundational training and education in nursing. NPs play a unique and complementary role in the healthcare system, practicing autonomously while integrating advanced clinical knowledge, diagnostic skills, and specialized expertise. Rooted in whole-person and patient-centered care, NPs consider the social determinants of health to provide comprehensive and individualized patient care.
