We’re Hiring: Nurse Practitioner – Black Health & Sickle Cell Care
Hamilton Urban Core Community Health Centre is seeking a dedicated Nurse Practitioner - Black Health & Sickle Cell Care (Extended Class) to provide holistic, culturally responsive primary care to Black individuals, families, and communities, with a specialized focus on Sickle Cell Disease. This role offers the opportunity to work within a dynamic interprofessional team, bridge hospital and community care, and address health inequities through trauma-informed, anti-racist practice.
You’ll support comprehensive primary care, community outreach, and the development of a community-based Sickle Cell clinic—making a meaningful, lasting impact on health and wellness.
