Please Note of new Changes in Procedures:

When arriving to your appointment to please read information posted on the door.

Please arrive at your appointment time and not earlier.

If the door is locked, the Nurse will come get you at your appointment time or as soon as the previous appointment is complete (That Includes clean up of areas and equipment).

There is Hand sanitizer provided in the waiting room, one person is permitted in the waiting room at a time. Mask is optional at this time but recommended.

All Donors/ Candidates will be asked to fill out the Covid-19 symptom Questionnaire upon arrival.

Be sure to have you government issued photo Identification ready prior to testing.

If this is a "Private Pay" Test please be aware that payment (Cash or company Check accepted only) will be required prior to testing.
