Wolf Creek PCN Welcome to the Wolf Creek PCN! We are part of your Patient Medical Home! PCNs develop solutions to meet the needs of the local community and their health issues.

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurse practitioners, nurses, dietitians, pharmacists, social workers, exercise specialist, behavioral health consultants, and mental health professionals. PCNs are created through an agreement between physicians and Alberta Health Services. Please note that PCNs are not a governing body to physicians; the College of Physicians & Surgeons of Alberta regulates the practice of medicine in Alberta. There are 40 PCNs operating throughout Alberta with more than 3,800 family physicians, and more than 1,400 other health practitioners. At any given time, there are hundreds of PCN family doctors across Alberta accepting new patients. Research shows that patients who regularly visit a family doctor are healthier and live longer. The Wolf Creek PCN supports the Central Alberta communities of Bentley, Delburne, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake. 🎃 When the tricks of Halloween fade, there's a treat on the other side: an extra hour of sleep.

Daylight Savings Time ends on Sunday, November 2.

If time changes usually throw you off, try these tips to help you adjust:

💤 adopt a calming ritual before bed and limit screen time
💤 ease in by gradually shifting your sleep schedule
💤 if you need to take a nap, keep it short and sweet
💤 spend time outdoors to enjoy natural light
💤 keep a consistent exercise routine
💤 skip caffeine when it gets close to bedtime

ADHD is a neurodevelopmental condition that affects people across all backgrounds, influencing attention, impulse control and emotional regulation.

💛 Awareness helps reduce stigma and opens the door to effective care. Behavioural strategies, parent training, structured environments and medical support can all play a role in helping youth thrive.

In Alberta, screening mammograms are available for people at risk of breast cancer starting at age 45, every two years until age 74.

No referral is needed, and you can self-book through the Alberta Breast Cancer Screening Program.

If you’re 40 to 44 and want to start earlier, or if you’re over 75, talk with your healthcare provider about what’s right for you.

Alberta Health Care cards are going digital.📱

The new Mobile Health Card can be accessed through the Alberta Wallet app or a web browser, offering a convenient and durable way to carry your health card!

Paper cards are still accepted but going digital means fewer lost or damaged cards, more security, and having your health card information always at your fingertips.

How to get your Mobile Health Card:

1️⃣ Ensure you have a verified Alberta.ca account and a MyHealth Records account
2️⃣ Apply for the mobile health card through your MyHealth Records account
3️⃣ Access it in the Alberta Wallet. The digital card will be stored in the new Alberta Wallet, which can be accessed on your web browser or through a mobile app

This month is Ovarian and Prostate Cancer Awareness Month. Awareness and early conversations with your doctor or primary care provider make all the difference.

🩵 Ovarian Cancer
Watch for symptoms like bloating, appetite changes, fatigue or frequent urination. If they last more than a few weeks, talk to your primary care provider.

💙 Prostate Cancer
1 in 8 men will be diagnosed in their lifetime. Men over 50 (or younger with family history) should ask their primary care provider about testing.
