Leduc Beaumont Devon Primary Care Network Integrated and comprehensive primary health care for a healthier Leduc County and its communities.

The Leduc Beaumont Devon Primary Care Network provides services for patients in the City of Leduc, towns of Beaumont and Devon and all communities in Leduc County. A group of 63 family physicians in 17 clinics along with additional health care professionals (registered nurses, registered dietitians, an exercise specialist, behavioural health consultants and mental health therapists) work together

to improve primary care for our patients. Primary Care Networks (PCNs) use a collaborative team approach to provide care for patients and to coordinate primary health care services provided by family physicians, Alberta Health Services and other health care professionals. PCNs are a way to provide team support for family doctors by bringing teams of health professionals together – doctors, nurses, dietitians, social workers, mental health professionals, pharmacists, occupational and respiratory therapists and others. PCNs also manage 24-hour patient access to appropriate primary care services.

Today is Advance Care Planning Day in Canada!
If you couldn’t communicate for yourself, even temporarily, who would make your health and personal care decisions? Would they truly understand what matters most to you?

Advance care planning is something you can do now, before a crisis or sudden change forces decisions to be made under pressure. It gives you time to think about your values, wishes, and priorities, and to talk them through with the people you trust.

Every adult over 18 should have a personal directive and enduring power of attorney. Attend this free workshop and leave with completed documents. The next session is May 7th - register early as this class fills up fast! Please see our website for more information: https://buff.ly/JtvUY4S

Caregiving can take many forms. It can look like helping a parent, supporting a partner, or caring for a child or family member. It’s often done quietly and over time, it can take a toll.
That’s why it’s important to check in with yourself and recognize the signs of burnout. Caregiver burnout is a state of physical, emotional and mental exhaustion resulting from prolonged, chronic stress. Key symptoms include:

➡️Constant fatigue
➡️ Sleep disruptions
➡️ Irritability and hopelessness
You don’t have to carry it all on your own. Your PCN is here to support you and the people you care for.
From mental health support to connecting you with local programs and resources, your PCN can help you stay on top of your own health while you care for others. Visit www.lbdpcn.com to learn more.
