A client came to see me complaining about SI joint pain on the R side 3 months post delivery. Due to her pregnancy and her surgery she was very inactive and her low back pain got worse over time. SI joint is supported anteriorly by the psoas and due to pregnancy and heavy load this support diminishes.
If your core is weak it can compress the SI joint to compensate and the structures posteriorly have to work harder. The SI joint is handling the load of transmitting force from the lower to upper body, and upper to lower.

In this specific case, she also mentioned that she holds her baby more on her R hip leaning more to the left. My assessment revealed a compressed R SI joint and weak under active core. Her glutes on the R also tested weak. Decompressed the SI joint and activated her core. Her homework includes core and glute exercises, and switching the hips while holding her baby.

My client who is a gymnast complained about a pain between the shoulder blades. Trigger point therapy did not work for her and actually caused more discomfort. She pointed to the rhomboid area. Upon the palpation of the Rhomboids i found no fascia restrictions. However the lat and the serratus anterior area were very tight. Rhomboids and serratus anterior both meet on medial border of scap and Rhomboids retract and serratus protracts the scap.

Often time serratus overworking and the rhomboid underworking. Did fascia work on the serratus and Lat then activated the rhomboid. After this treatment she reported she was pain free for a month after her initial treatment.

My regular client had knee pain few months ago on the R side and got better over time. He saw me today complaining about back pain after few rounds of golf. The Kemp test useful for diagnosing pain related to facet joint pathology was negative. However he tested positive for Ely test which is hip flexor tightness. Since golf is a combination of upper and lower body movement, due to lack of stability from lower part like the R knee in this case the compassion patterns can been seen on upper body like lower back pain.
Released the glute muscle on the L and activated hip flexors on the R. After several reps of treatments client felt better and sent home with his homework till next treatment.

My client complained that when he lays down or when his little daughter falls asleep on his shoulder his whole arm goes numb, and his shoulder hurts. He pointed to the brachial plexus area where the Thoracic Outlet resides.

In that area, the biceps and pecs meet where all the muscle imbalances of the shoulder can be affected.

Based on my assessment, I found the pec minor was tight and overworked, causing impinging the brachial plexus. Even though he works out a lot, his lat was tight and weak ( again due to muscle imbalances of his shoulder). Released his pec minor and activated the lat and infraspinatus.

His homework was proper form of exercises for lat and infraspinatus strength.

Maybe You Shouldn’t be Stretching Your Hip Flexors?

Often time a dysfunctional psoas can cause lower back pain because of its attachment to the diaphragm and lumbar area. Since so many people sit too much the psoas has become shortened and weak. Stretching a weak and tight psoas is not effective because often time the activation exercises is the answer.

Why stretching does not work?

👉First, stretching does not reeducate the nervous system.
👉Second, when you pull on an already tight muscle the stretch reflex is activated, making the muscle contract even more.

Activation exercises send biofeedback to the brain regarding the level of contraction in our muscles, thereby helping to prevent the buildup of chronic muscular tension.

The only way to change the resting level of muscle tension being set by the nervous system is through active movement.
