Retreatment case on a patient who was not a good candidate for surgery due to the risk of MRONJ. The apical anatomy posed a challenge due to a lateral root perforation after zipping the canal. The prior root filling was completely removed and the original anatomy was re-established with small curved hand files. Irrigation finished the chemomechanical prep.

Unfortunately, I needed to reseat the GP cone several times, which contributed to the extrusion of RRM. We’ll see what the 6-month follow up shows.

Premolar was restored with a quartz fiber post.
