Same disc bulge. Completely different pain levels. Why?

This is one of the most common questions I get — and one of the most important ones to understand.

It comes down to four things:
1. Nerve proximity — the direction and position of the bulge relative to the nerve root makes a massive difference.
2. Spinal canal size — some people have more natural space; others have less room before compression occurs.
3. Muscle function — strong spinal stabilizers protect the nerve by absorbing load. Weakened stabilizers leave the nerve exposed.
4. Nerve sensitivity — a nerve that's been irritated over time becomes hypersensitive, amplifying pain signals even from mild compression.

This is why we never treat the image — we treat the person.

When your back is flaring up, the instinct is to rest. Stop moving. Lie down. Wait for it to calm down.

And for the first 24 to 48 hours after an acute injury, that's often the right call. Your body is dealing with initial inflammation, and giving it a window to settle is reasonable.

But here's where it goes wrong for a lot of people. After that 48-hour window, continued rest starts working against you.

Here's why. Your spinal discs don't have a direct blood supply the way muscles do. They get their nutrients through movement — through the gentle compression and decompression that happens when you walk, move, and change positions. When you stop moving, that fluid exchange slows down. The disc starts to dehydrate. The supporting muscles weaken and stiffen. And instead of calming down, the pain cycle deepens.

There's also a neurological component. When you avoid movement due to pain, your brain starts to associate movement with danger — even when the danger has passed. This is called central sensitization, and it's one of the reasons some people stay in pain long after the original injury should have healed.

This is something we see constantly at Cameron Family Chiropractic. People come in after weeks of rest, thinking they did everything right — and they're often worse off than if they'd started gentle, guided movement earlier.

The 48-hour rule is this: rest is appropriate in that initial window. After that, the goal shifts to controlled, supported movement — not more rest.

Here's what most patients are never told after seeing their MRI results:

The size of a disc bulge is one of the least reliable predictors of how much pain someone is in. We regularly see patients with significant findings who feel fine — and patients with mild findings who are debilitated.
What actually determines your pain level:

🔹 Nerve proximity — a bulge pressing directly on the nerve root causes intense symptoms. The same bulge pointing a slightly different direction may cause none at all.
🔹 Spinal canal size — narrower canals leave less room for the nerve to escape compression. This is anatomical — you're born with it.
🔹 Stabiliser muscle function — when the deep muscles supporting your spine are working well, they absorb load and protect the nerve. When they've shut down from injury or chronic pain, that protection disappears.
🔹 Nerve sensitivity — nerves that have been compressed or irritated for a long time become centrally sensitised. They fire pain signals with increasing intensity from decreasing provocation.

Before you accept that your scan is the final word on your condition — know that the real picture is far more detailed than any image can show.

Your MRI isn't the whole story.

A disc bulge on a scan can mean very different things for different people. Some people have significant disc changes and feel nothing. Others have a "mild" bulge and are in serious pain.

Why the difference? Because an MRI is a static structural image — it doesn't show how your spine is moving, how the nerve is responding under load, or how your supporting muscles are functioning.

The difference comes down to four factors that a scan simply cannot show:

1️⃣ Nerve proximity — the direction of the bulge determines whether it's pressing directly on the nerve root or missing it entirely.
2️⃣ Spinal canal size — some people are born with more space around the nerve. Others have a narrower canal where even a small bulge causes significant compression.
3️⃣ Muscle function — deep stabilising muscles like the multifidus protect the nerve by absorbing spinal load. When those muscles shut down from previous pain or injury, the nerve takes the full impact.
4️⃣ Nerve sensitivity — a nerve that's been irritated over time becomes hypersensitive. It amplifies pain signals even from mild compression — which is why some people with minor findings on their scan are in serious pain.

Lower back pain that shoots into your leg can be incredibly frustrating. You try stretching, you try exercising, and you genuinely want to do the right thing for your recovery. Here are 3 signs that prolonged rest is making your back pain worse — not better:

1️⃣ The pain feels deeper and more constant — not sharp and reactive like it did at first. This is a sign the surrounding muscles have weakened and the disc is losing hydration from lack of movement.
2️⃣ You feel stiff and sore after sleeping or sitting — your stabilising muscles have started to switch off. They no longer activate the way they should to protect the spine.
3️⃣ Movement now feels scarier than it did before — this is central sensitization at work. Your nervous system has learned to associate movement with danger, even when the original injury has settled.

None of these mean you're getting worse permanently. But they do mean rest alone is no longer the right tool.
