Our team was happy to represent at the Association of Family Health Teams of Ontario's 2025 Conference! It was a great opportunity to connect and share insights with colleagues dedicated to advancing primary care in Ontario.
Highlights from this year’s conference:
▪️Leadership in Practice: Exploring how transparency, accountability, and balanced decision-making shape stronger more trusted governance in healthcare.
▪️Collective Intelligence: Insights from Canadian Biologist & TV Producer Daniel Riskin on how teams can grow and thrive together.
▪️Patient Attachment: Meaningful discussions on improving access and continuity of care — ensuring every patient has a home in the primary care system.
▪️Learning how Six Nations of the Grand River FHT blends Western and traditional medicine in the Haudenosaunee Wellness Model, achieving 89% patient and 100% staff satisfaction.
▪️Exploring how St. Michael’s FHT practices equity-based care through social prescribing and the SEED program (Support, Equity, Engagement, Dignity).
