Today, Dr. Marie Gdalevitch begins a tibial lengthening for a patient who previously underwent femoral lengthening.

The procedure starts with a careful assessment of muscle flexibility, followed by precise anatomical marking — including Dr. Gdalevitch’s signature smiley face over the kneecap 😉

An anterior fasciotomy is then performed to release the fascia and create space for safe, gradual lengthening. Once the fascia is opened, the pathway is confirmed from top to bottom to ensure alignment and safety.

Next, we identify the ideal angle for the locking screw, starting laterally on the fibular side and exiting medially. During this step, a 𝐬đČ𝐧𝐝𝐞𝐬𝐩𝐹𝐭𝐱𝐜 đŹđœđ«đžđ° 𝐱𝐬 đšđ„đŹđš đ©đ„đšđœđžđ 𝐭𝐹 đŹđ­đšđ›đąđ„đąđłđž 𝐭𝐡𝐞 𝐭𝐱𝐛𝐱𝐚 𝐚𝐧𝐝 đŸđąđ›đźđ„đš, maintaining proper alignment throughout the lengthening process.

This careful sequencing and fixation ensure stable, controlled lengthening and optimal functional recovery over time.

Had a fantastic time teaching at the Conmed Foot and Ankle MIS Skills Lab this past Saturday in Toronto, alongside Dr. Warren Latham.

We welcomed surgeons from across the country to learn and refine minimally invasive foot and ankle surgery (MIFAS) techniques for treating a variety of pathologies. It’s always inspiring to see so much enthusiasm for advancing patient care and surgical innovation.

Just returned from an incredible LLRS meeting in Philadelphia, where I had the chance to reconnect with old friends and make some amazing new ones.

This meeting was more than just clinical—it was about becoming better humans. From powerful discussions on self-care to the awe-inspiring talk by Paralympic athlete (who literally brought us to our feet), I left feeling recharged and motivated.

What makes our society so special is how it brings together diverse minds united by a common goal: to keep pushing boundaries and improving our field.

Thank you to everyone who made this meeting so memorable.

I truly can’t imagine a more meaningful way to celebrate my 45th birthday. Spending the week on a humanitarian mission at Nepal Orthopedic Hospital with Heal the Children USA was an experience I’ll never forget.

Huge thanks to our incredible team leader, Dr. Ali Khosroabadi, and his amazing wife Karin, who managed every detail behind the scenes. I was also honored to share this mission with my longtime mentor and friend, Dr. John Herzenberg, and the inspiring Dr. Jennifer D’Amico, who began this partnership 15 years ago.

We were proud to represent Montreal with our Verdun Hospital “Fantastic Four”: our amazing nurses Christelle Vincent and Marie-Pier Caron, who seamlessly integrated with the local team, and our adaptable anesthetist Dr. RaphaĂ«lle Trudeau, who worked magic to keep patients safe and comfortable.

On Day 1, we saw over 200 patients in clinic. We then triaged and performed 35 surgeries—ranging from routine to highly complex—each one aimed at restoring function and relieving pain for those with limited access to care.

None of this would’ve been possible without our tireless local hosts—Dr. Rachit, Dr. Nepal, and Dr. Bijendra—who worked alongside us and will continue patient follow-up. Their dedication ensures long-term care and lasting impact.

We closed the week with a teaching session for local orthopedic surgeons and residents—capping off a week full of learning, collaboration, and shared purpose.

Heartfelt thank you to Orthopediatrics and Conmed for their generous donation of orthopedic implants and equipment!

The Nepalese people welcomed us with warmth, generosity, and their beautiful “Namaste” spirit. We return home tired but fulfilled—grateful for the chance to help, grow, and build lasting bonds.

Rien de tout cela n’aurait Ă©tĂ© possible sans nos hĂŽtes locaux gĂ©nĂ©reux et dĂ©vouĂ©s — Dr Rachit, Dr Nepal et Dr Bijendra — qui ont travaillĂ© Ă  nos cĂŽtĂ©s pendant de longues journĂ©es en salle d’opĂ©ration et qui assureront le suivi mĂ©dical.

Nous avons terminĂ© la semaine par une sĂ©ance d’enseignement pour les chirurgiens orthopĂ©distes et rĂ©sidents locaux, clĂŽturant une semaine riche en Ă©changes, apprentissages et collaboration.

Le peuple nĂ©palais nous a accueillis avec chaleur, gĂ©nĂ©rositĂ© et ce bel esprit du “Namaste”. Nous rentrons fatiguĂ©s mais comblĂ©s, reconnaissants d’avoir pu aider, apprendre et tisser des liens prĂ©cieux.

A heartfelt thank you to the British Limb Reconstruction Society (BLRS), and especially to Steve Borland and Bree Winger, for the honor of being invited as a keynote speaker this year in Newcastle Upon Tyne. I’d also like to extend my gratitude to Clockwork Medical for their exceptional organization of the event.

It was truly an honor to be among such a distinguished group of surgeons, and to share the stage with Nando Ferreira and Jan Duedal Rölfing. The standard of limb reconstruction surgery in the UK and the quality of the presentations were nothing short of outstanding. The BLRS members exemplified the power of a cohesive and welcoming community, highlighting the need for unity in our specialty across the globe.

In addition to the incredible learning experience, I’m grateful to have made new friends and colleagues. I look forward to staying connected and to seeing everyone again soon. I also want to express my gratitude that my twin sister, PHI Plastic Surgery / Dr Perry Gdalevitch, was able to join me on this trip – her presence made the experience even more special.

I leave this meeting with a wealth of new knowledge and a renewed determination to continue advocating for the importance of this specialty in Canada and North America.
