Why treating the chin isn't always about just adding filler
This patient came in wanting to improve the overall balance of their lower face. After a careful assessment, two structural contributors were identified that were impacting that harmony:
• A chin with limited natural projection
• A very active mentalis muscle, creating dimpling
and a slightly puckered appearance even at rest
When the chin lacks projection, the mentalis muscle has to work overtime to compensate, which is what can create a textured, strained look around the mouth.
If we had gone straight in with filler alone, the result would have been limited (and often still dimpled). Instead, treatment was done in two stages to address both the cause and the compensation:
Step 1: Neuromodulator to relax the overactive mentalis
→ This softens the puckering and reduces downward pull
Step 2 (2 weeks later): 4 syringes of HA fillers were strategically placed
→ Restores projection, improves support, and balances the lower face
By combining these treatments, we're not just changing the appearance, we're helping to correct the underlying mechanics.
The result:
• Smoother chin texture at rest
• Improved projection and profile
• Better harmony between the lower face and lips
This is why full-face assessment matters. Treating in isolation can miss the bigger picture.
