Kingston Community Health Centres was well represented at the Living Well with Heart Failure and COPD community event on April 22, hosted by the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT).

This free event brought together people living with COPD and heart failure, caregivers, and community members to explore practical ways to manage symptoms, connect with local supports, and learn from clinical and community experts. With more than 20 local programs and service providers on site, the event highlighted what coordinated, community-based care can look like in action.

Thank you to everyone who attended and to our team members who helped make the day such a success.

Here are a few moments from the event.

Sammie brings her lived experience and passion to the PRISM drop-in group in Napanee, supporting 2SLGBTQIA+ youth and their allies.

Having experienced homelessness as a teenager, she understands just how important safe, welcoming spaces can be. Now 24 and a Community Service Worker graduate, Sammie volunteers her time to support group activities and show up as a relatable, empathetic presence.

“This program is exactly what Napanee needs,” she says. “You can tell it’s a place where they feel most comfortable to be themselves. They look forward to it.”

Sammie also works with Resolve Counselling in Kingston, continuing her commitment to helping others navigate challenges and giving back to her community.

We’re so grateful to have Sammie as part of our team 💚
