Exactly one month since my return from gallivanting in sunny Tanzania, back in freezing England, and already had my first weekend on call, and like many before, it did not disappoint. Considering my last weekend on call was over 6 months ago at much smaller District General Hospital which did not even have an emergency department. And now I am at a much larger hospital than the previous, as a more senior registrar, meaning I get a minion, that looks up to me, and I actually have to know what I am doing all the time....

This week, I returned to work after three weeks holiday. As I was doing my usual customary walk-abouts round our ten theatres, I noticed one of the registrars preparing for intubation. I decide to walk in to greet him, I shout “Heri ya mwaka mpya” (Happy new year) and ask if he needs any help. He had 3 medical students and was about to intubate....

A phrase of encouragement I learnt from my taxi driver who quickly became my friend. In every situation, African people have an admirable way of seeing the positive. Today (well, 5th of August) marks one year since the inception of this blog. I feel as though I have achieved a lot. I have now completed my ST4, which is specialist registrar of the 4th order, of a possible 7 occasionally 8, and with it my intermediate training. I have been fortunate enough to complete all my fellowship exams in anaesthesia, and all this allows me some freedom to shape and dictate, to a point, the direction of my next training years....
