The Hospice Hub would like to share our heartfelt gratitude to Donna from Rideau Lakes, whose kindness and generosity have truly warmed our hearts. Donna has lovingly knitted over 100 beautiful mini hearts for our Hearts for Hospice Campaign. Each tiny heart represents compassion, care, and the spirit of community that helps support hospice programs and services for individuals and families when they need it most.

Donnaโ€™s incredible dedication reminds us that even the smallest acts of kindness can make a big impact. ๐Ÿ’™ Thank you, Donna, for putting so much love into every stitch.

We would love for more community members to join this meaningful initiative! If you knit or crochet, consider creating your own Hearts for Hospice to help spread comfort and support throughout your communities in Perth, Smiths Falls, Lanark, Rideau Lakes, Westport and surrounding areas.

The Hospice Hub is recruiting for a Registered Nurse to fill a full-time Nurse Coordinator Home Support and Day Hospice position (35 hours per week) for a one year renewable term. The successful candidate is responsible for facilitating and coordinating hospice palliative care for clients and families at end-of-life and clients with a serious illness.

The overall responsibilities include: assessing the needs of the clients; developing care plans; selecting home visiting volunteers, as required; and providing healthcare navigation support to clients and families.

The Nurse Coordinator oversees the Day Hospice Program in Smiths Falls. In addition, the Nurse Coordinator will lead a small working group to implement a new Day Hospice Program in Perth.

Hearts for Hospice is officially live!

Weโ€™re inviting the amazing crafters in our community to knit or crochet handmade hearts, which will be shared for a donation in support of THHโ€™s services and programs. Any colour, any pattern, and any size (under 4") will be gratefully accepted.

All funds raised support THHโ€™s community programs and services which are provided at no cost to participants.

Drop off locations include Smiths Falls Public Library, Lanark Highlands Public Library, Westport Ontario Public Library, Perth Fabrics and Crafts and Rideau Lakes Public Library branches: South Elmsley, Portland and Elgin. Interested in becoming a drop off location? Please reach out!
