ConnectWell Community Health - Lanark Site

On April 6th, the Lanark site of ConnectWell Community Health will be replacing the generator. The power to the building will be turned off and therefore we are closed for in person appointments.

The phones will continue to be answered, and staff are available by phone to speak with you. Please call 613-259-2182 to speak with a ConnectWell staff.

We will return to regular hours on Tuesday, April 7th.

Friday, April 3rd: Office closed for Good Friday.
Monday, April 6th: Office closed, phones answered 8:00 AM-5:00 PM
Tuesday, April 7th: return of regular hours

Social prescribing bridges the gap between clinical and social care.
Health care providers connect clients with a link worker, who connects with the client to understand their needs and interests.
