Social prescribing bridges the gap between clinical and social care.
Health care providers connect clients with a link worker, who connects with the client to understand their needs and interests. The link worker connects the client with a range of community supports and follows up. People are empowered to develop their interests, goals and gifts while connecting with their community. Speak with your healthcare team to learn more.
