As part of providing you with the safest and most effective care, I ask all clients to update their health history form once per year — even if nothing has changed.

Life evolves, and small updates (medications, injuries, stress levels, surgeries, or wellness goals) help me tailor each session to best support your body and overall wellbeing.

If it has been a year or more since your last update, please take a moment to complete a new form before your next appointment. It truly makes a difference in the care I’m able to provide.
