ANVH EP-77 Xylitol poison in dogs 狗的木醣醇中毒

Today, A Niu would like to share a case of Xylitol poisoning in dogs

Xylitol is extremely toxic to dogs. Even small amounts can cause life-threatening hypoglycemia (low blood sugar), seizures, liver failure, or death. If a dog ingests xylitol, immediate veterinary care is critical.

Why Xylitol Is Dangerous for Dogs
Sugar alcohol: Xylitol is a sugar substitute found in sugar-free gum, candies, baked goods, peanut butter, toothpaste, mouthwash, and even some medications.
• Insulin surge: In dogs, xylitol triggers a rapid release of insulin, leading to a sudden and profound drop in blood sugar (hypoglycemia) within 30 minutes to 2 hours.
• Liver damage: Larger doses can cause acute liver injury or failure within 24–48 hours.
• Species-specific: Cats, ferrets, and horses are not affected in the same way; dogs are uniquely sensitive.

Symptoms of Xylitol Poisoning: Vomiting/Weakness, lethargy, or collapse/Ataxia (loss of coordination)/Seizures/Coma
In severe cases: jaundice, bleeding disorders, liver failure

Note: The amount of xylitol varies widely between products. Even one piece of sugar-free gum can be dangerous for a small dog.

🩺 What To Do If Your Dog Eats Xylitol
• Do not wait for symptoms. Call your veterinarian or an emergency clinic immediately.

Bottom line: Xylitol ingestion is a medical emergency in dogs. If you suspect your dog ate anything containing xylitol, seek veterinary help immediately — even if they seem fine at first.

木糖醇對狗來說是劇毒,他會造成肝臟過度分泌胰島素而造成嚴重低血糖,涉入的量太多則引發肝衰竭,而且只有狗這個物種會對木糖醇有毒性反應

常見的臨床症狀有嘔吐,虛弱,倦怠,食慾下降,嚴重的話會有癲癇,黃疸,肝衰竭等症狀

如果你的狗吃了木糖醇,請趕快跟你的家庭獸醫聯繫,即使他看起來是很正常的,記住,木糖醇對狗來說是一種劇毒

這個case在吃下口香糖後幾小時內肝指數上升到600多(正常125以下),血糖掉到30幾(正常120-150),住院點滴治療幾天後慢慢恢復正常

ANVH EP-76 Statement related to the false accusation

Recently, A Niu has received several malicious negative comments on Google, accusations such as :

1. A Niu is an “Extortion, retaliation, and zero ethics.” animal hospital.
2.“They refused to discuss the results or the sudden price surge. They demanded $60+ just to answer basic follow-up questions a week later.”
3. “Delete the negative Google reviews or your dog is permanently banned from our clinic.” That’s not healthcare. That’s straight-up blackmail. Etc.

Toward this false accusation, our official response is below:
The owner is lying; what they claim is not true, not even close. We would like to clarify that the appointment was originally a recheck for a pre-existing condition, but new clinical signs were reported by you during the visit.

Because of this, additional blood work was recommended. The estimate was presented, reviewed, and the quotation was obtained before we proceeded. That means the owner definitely knows what we will do and gives us permission to do it. The exam results, comparisons to previous findings, and recommendations for diet and supplements were all explained during that consultation.

No concerns were raised at the time. We only became aware of the owner’s dissatisfaction a week later, following with one-star review related to the $50 recheck fee. Since the report had already been fully explained previously, it is appropriate to charge a recheck fee when using additional doctor time to re-explain the same results—especially to someone who was not present at the original visit.

It looks like many misunderstandings stem from this second-hand interpretation. At A Niu Veterinary Hospital, we do not pressure or threaten clients. All estimates and invoices are reviewed with clients before any treatment or medical behaviour is provided. The statements in the owner’s review significantly misrepresent the events that occurred and have negatively impacted our hospital’s reputation. We will be seeking legal action.

Given the communication barriers and the breakdown of trust, we believe we are no longer the right fit for this owner’s veterinary needs. We are happy to transfer the pet’s medical records to any hospital of owner's choice.

1. 阿牛是一間”勒索,報復,無道德的醫院”
2. 拒絕說明跟解釋收費內容,還要求更多的門診費用
3. 刪掉負評不然A Niu就拒絕診療等等,以及還有許多不實的指控

針對這些不實的指控,阿牛的回應如下:

飼主講的並不是事實。我們想澄清，這個主人預約原本是針對既有病況的複診，但在就診過程中提出了新的臨床症狀像是黑便跟多喝水。

因此，楊醫師建議進行額外的血液檢查來排查有無其他問題。檢查所需的費用報價已在進行前呈現、說明並由飼主簽署報價確認後才進行。所有檢查結果、與先前數據的比較，以及飲食與保健品的建議，都已在當次門診中詳細解釋。

門診結束時並未收到飼主的疑慮。我們是在一週後，因飼主針對 50 元加幣複診費留下的一星評論，才得知飼主的不滿。由於報告內容先前已完整說明，若需額外花費醫師時間再次解釋相同結果，尤其是對未參與原始就診的人，收取複診費是合理的。

看來許多誤解源自於這種間接的轉述。在阿牛動物醫院，我們絕對不會，也不可能強迫或威脅飼主。所有預估費用與帳單都會在任何治療或給藥前，與客戶確認並同意後執行。我們絕對是收費公開透明的動物醫院。這個主人的評論內容嚴重扭曲跟背離事實，並對本院的聲譽造成負面影響，我們會採取法律的動作。

鑑於溝通障礙與信任破裂，我們認為已不再適合繼續為這個飼主提供任何醫療服務。我們會將寵物的醫療紀錄做好轉給飼主選擇的任何醫院。

ANVH EP-75 Uroliths in a female dog - 母狗的膀胱結石

Today A Niu would like to share a case of uroliths in a female dog.

Uroliths, also known as bladder stones, are hard mineral deposits that form in the urinary bladder of dogs. They can range in size from tiny grains to larger stones and are often caused by factors like urinary tract infections, dietary imbalances, or abnormal urine pH.

Common symptoms include:
Blood in the urine.
Straining or difficulty urinating.
Frequent urination in small amounts.
Pain or discomfort in the abdomen.

Treatment options depend on the size and type of the stones. Small stones may pass naturally, while larger ones might require surgical removal or other medical interventions.

If you suspect your dog has bladder stones, it's best to consult a veterinarian promptly.

在狗,膀胱結石表示礦物化的沉積物累積在膀胱內,形成像石頭一樣構造的東西;結石可以有很多不同種類跟大小,常見的形成原因有尿路感染,飲食習慣,先天體質等眾多因素造成

常見的臨床症狀為有頻尿,血尿,排尿時有疼痛灼熱感,可能會哀鳴or排尿困難,腹痛,精神活力下降等等

臨床上經過影像的檢查可以確認有沒有結石的生成,通常小於0.5cm的特定結石種類可以靠處方飼料溶解或變小,但體積龐大的結石則必須考手術移除

這個病例是一個母狗,近幾周以來頻尿,有時候帶血,經過2周的保守治療不見好轉,和主人討論後決定手術取出
通常公的動物,結石都是小顆但是數量很多,母的都是一大顆結石塞滿膀胱,這個病例是後者

ANVH EP-73 Chylothroax in dogs and cats - 犬貓的乳糜胸

Today A Niu would like to share a rare case of Chylothorax in dogs and cats

🩺 What Is Chylothorax?
• Chylothorax is a type of pleural effusion where chyle—a milky fluid rich in triglycerides—leaks abnormally into the pleural cavity, compressing the lungs and causing breathing difficulties.
• Chyle is formed from dietary fat absorbed in the intestines and transported via the lymphatic system through the thoracic duct. If the duct ruptures or becomes obstructed, chyle can leak into the chest cavity.

Commonly Affected Breeds and Causes
• Dog breeds: Shiba Inu and Afghan Hound are predisposed.
• Cats: Can occur at any age.
Common causes:
• Heart disease
• Thoracic tumors
• Lung lobe torsion
• Diaphragmatic hernia
• Venous thrombosis
• Trauma or surgical injury to the thoracic duct
• Idiopathic chylothorax: No identifiable cause; accounts for most cases.

⚠️ Clinical Signs
• Rapid breathing, coughing
• Lethargy, loss of appetite
• Exercise intolerance, hypoxemia
• Thoracocentesis yields milky or pinkish fluid (if blood is mixed)

Diagnostic Methods
• Thoracocentesis: Fluid a**lysis for triglyceride concentration (higher than serum) and protein levels.
• Imaging: Chest X-rays, CT lymphangiography, echocardiography.
• Blood tests: To rule out heartworm, infections, electrolyte imbalances.

Treatment Options
• Preferred surgical approach: Thoracoscopic thoracic duct ligation ± pericardiectomy.
• Severe cases: May require cisterna chyli ablation.
• New techniques: ICG fluorescence imaging helps locate the thoracic duct and improves success in minimally invasive surgery.
• Prognosis: Idiopathic cases have a surgical success rate over 90%. Cases with underlying disease require comprehensive management.

• 胸腔穿刺液呈乳白或粉紅色（混血）。

診斷方式
• 胸腔穿刺：抽取液體檢測三酸甘油脂濃度（高於血清）、蛋白質濃度。
• 影像學：X光、CT淋巴管造影、心臟超音波。
• 血液檢查：排查心絲蟲、感染、電解質紊亂等。

治療方式
• 首選手術：胸腔鏡引導下胸導管結紮術 ± 心包膜切除術。
• 部分嚴重病例：可能需乳糜池摘除術。
• 新技術：ICG螢光顯影輔助胸管定位，提升微創手術成功率。

ANVH EP-71 IVDD in dogs 狗的椎間盤疾病

IVDD (Intervertebral Disc Disease) in dogs is a spinal condition that can cause pain, mobility issues, or even paralysis, especially in breeds with long backs and short legs. Early detection and treatment are crucial.
Here’s a comprehensive overview of IVDD in dogs:

What Is IVDD?
IVDD occurs when the intervertebral discs—the cushioning pads between the vertebrae—degenerate or rupture, putting pressure on the spinal cord and nerves. This can lead to symptoms ranging from mild discomfort to complete paralysis.

Breeds at Risk
Dogs with long backs and short legs are especially prone to IVDD, including:
• Dachshunds (25% may experience IVDD in their lifetime)
• Basset Hounds
• Corgis
• Shih Tzus
• French/English bulldog
These breeds often have a genetic predisposition that weakens spinal discs over time.

Types of IVDD
There are two main types:
• Hansen Type I: Sudden rupture of the disc, often seen in chondrodystrophic breeds (short-legged dogs). The disc material becomes calcified and can shoot out, causing acute pain or paralysis.

椎間盤疾病是指脊椎方面的問題造成疼痛,移動障礙,嚴重的case甚至癱瘓,特別是在一些特殊的犬種

椎間盤疾病通常是因為在脊椎骨中間的緩衝組織(椎間盤)發生腫脹,破損而壓迫到神經,進而引發的症狀,神經方面通常會引起較為劇烈的疼痛跟癱瘓,排便尿也會受到影響(神經控制)

好發犬種為:臘腸,柯基,西施,法國/英國鬥牛犬,巴吉度獵犬等
這些犬種因為基因上的缺陷,而特別容易出現這些問題,其中25
%的臘腸狗在一輩子狗生當中一定發生過IVDD

IVDD有分成三種類型:
1. Hansen type I:椎間盤突然的破損,常見於短腿體型的狗,椎間盤物質溢出且壓迫到神經,造成急性且劇烈的疼痛,有可能伴隨癱瘓或排便尿困難
2. Hansen type II: 椎間盤的退化腫脹造成神經的壓迫,較常發生於大型犬種,引起慢性疼痛,這問題通常伴隨著數月到數年的時間
3. 創傷造成,專間盤暫時性的水腫/瘀血造成壓迫,經過治療後恢復正常,通常不會留下後遺症

臨床症狀有:跛行,拱背,脖子僵硬,無法移動,劇烈的疼痛,癱瘓等
常見的檢查有理學檢查跟X光,想要確診部分必須做高階的影像學檢查

那我們怎麼治療?
口服消炎止痛藥,限制活動強迫休息,嚴重的case必須進行手術,接著是漫長的復健之路

這個疾病預防勝於治療,盡量避免垂直的運動,保持健康的體重,睡的床要有適當的支持,以及早期就醫

ANVH EP-70 Degloving trauma - tail amputation 脫套性傷害 - 截尾手術

A Niu received a phone call from an ambulance driver, said they found a dog with degloving trauma in the car accident scene...

Today A Niu would like to introduce a case of degloving trauma.

Degloving trauma in dogs is a severe injury where skin and underlying tissues are torn away, often due to being dragged or hit by a vehicle. It requires intensive wound care and sometimes multiple surgeries, but with proper treatment, recovery is possible.

Degloving injuries occur when a dog’s skin and subcutaneous tissue are forcibly separated from the underlying structures due to shear forces.
Common causes include motor vehicle accidents, especially when the dog is dragged or pinned.
The distal limbs (especially the medial tarsus/metatarsus) are most frequently affected.

🩺 Severity and Associated Damage:
Loss of skin, muscle, tendon, ligament, and bone
Neurovascular damage
Joint capsule pe*******on and instability
Embedded debris and bacteria, making the wound highly contaminated.

Treatment Approach:
Initial stabilization is critical—address life-threatening issues like blood loss before wound care begins.
Wound management
Multiple surgeries may be required depending on severity

上週,阿牛接到一通來自救護車司機打來的電話,說現場有一隻脫套性傷害的狗,問可不可以現在送來

今天阿牛要介紹的病例是"脫套性傷害"

脫套性傷害是指皮膚跟下面的軟組織嚴重的分離(脫手套一樣),常見於車禍,或其他機械性的創傷(剪力把軟組織捲入,造成嚴重的撕裂傷)
脫套性傷害通常位於肢端末段,而且傷口都很髒或有感染的風險

治療上通常第一步是先穩定病患,接著清理傷口,根據動物受傷程度可能需要多次的手術來修復手傷的組織,當遇到不可逆的傷害時則考慮手術截除

這個case是典型的尾巴脫套性傷害,不到一歲的狗在高速公路上突然跳車,造成了交通事故,幸運的是狗狗的生命現象穩定,經手術截除尾巴後恢復良好

ANVH EP-69 Abscess(膿瘍)

Today, A Niu would like to share a case of severe abscess in a golden retriever.

An abscess in a dog is essentially a painful pocket of pus caused by infection, trauma, or foreign objects. These can appear under the skin, in the mouth, or even internally. Common causes include bite wounds, splinters, or infected a**l glands. Symptoms often include swelling, redness, warmth, and sometimes a foul-smelling discharge. Dogs may also show signs of pain, fever, or behavioral changes like lethargy or loss of appetite.

Treatment usually involves draining the abscess and administering antibiotics. In some cases, surgical intervention may be needed.

This case is a 4-year-old golden retriever, came with an extremely swollen face. Initially, we think about the tumor or autoimmune issue. After examination, diagnosed with a huge abscess around the chin/R face area.

Our team performed a surgery to drain out more than 200ml pus, and put a draining tube into the wound. The dog improved a lot in 3 days.

膿瘍是指在動物身上某個部位有一個囊袋狀的構造且裡面充滿膿汁,常見於創傷,感染or異物穿刺等等;他可以發生在動物的任何部位,在犬貓最常見的原因是打架
患部會有瘀青or紅腫,嚴重的病例會疼痛跟發燒(像這個case)

引流出膿汁是治療的第一要務,配合抗生素的治療,嚴重的病例需要手術介入(也是這個case)

在這隻4歲的黃金獵犬,整個右臉部跟下巴明顯腫脹,狗發燒到40度,不吃不喝;一開始還以為是腫瘤或者自體免疫的疾病,在經過檢查後,診斷為膿瘍

我們的團隊隨即手術介入,引流出超過200ml的膿汁,我們放了引流管進去傷口,3天後回診,腫脹的傷口已經好了很多,並移除引流管

ANVH EP-68 C-section in cat(貓的剖腹產)

A cat that walked into the hospital, and the owner said she had delivered a small kitten within an hour. Unfortunately, the first kitten died, and there's another one inside the mom...

A C-section in cats, or cesarean section, is a surgical procedure performed when a cat (queen) cannot deliver her kittens naturally due to complications like dystocia (difficult labor), fetal malposition, or uterine inertia. It’s often an emergency intervention to save both the mother and her kittens.

🩺 When Is a C-Section Necessary?
• Prolonged labor without delivering a kitten
• Gestation exceeding 65 days
• Fetal distress or death
• Maternal issues like a narrow birth ca**l or uterine rupture

After a quick ultrasound to confirm the fetus's status, a C-section was performed to help the mom deliver another kitten successfully.

Cesarean section is an urgent surgery. The earlier exam and diagnosis, the better prognosis for mom and the babies.
That's why timing is critical.

剖腹產是一種外科手術，應用在貓狗無法自然生產的情況下，像是難產/胎位不正確/子宮無力等等

什麼時候該找獸醫判斷有無剖腹產的需要呢?
1. 生產時間超過一小時
2. 懷孕時間超過65天
3. 每隻胎兒間隔超過1小時以上
4. 死胎或畸胎
5. 母體的異常像是產道狹窄或子宮破裂等等
