ANVH EP-81 Parvovirus alarm in Vancouver 犬細小病毒爆發警報

Canine parvovirus (CPV) is a highly contagious and often life‑threatening viral disease in dogs, especially puppies. It causes severe gastrointestinal illness, rapid dehydration, and can be fatal without prompt veterinary care.

CPV Is
• A highly contagious virus that spreads through infected f***s and contaminated environments.
• Primarily affects puppies 6–20 weeks old, but unvaccinated adults can also get sick.
• Causes acute hemorrhagic gastroenteritis and sometimes myocarditis in very young pups.
• The virus mutates quickly, leading to new variants that continue to circulate globally.

How It Spreads
• Direct contact with infected dogs
• Contact with contaminated surfaces (floors, soil, shoes, bowls, hands)
• The virus survives for months in the environment and is resistant to many disinfectants.

Clinical Signs
Common symptoms include:
• Severe vomiting
• Profuse, often bloody diarrhea
• Lethargy, fever, anorexia
• Rapid dehydration
• Abdominal pain

Any puppy with these signs should be treated as an emergency.

Treatment
There is no antiviral cure, but aggressive supportive care dramatically improves survival:
• IV fluids and electrolyte correction
• Antiemetics
• Antibiotics to prevent secondary bacterial infection
• Nutritional support
• Isolation to prevent spread
• Some clinics now use monoclonal antibody therapy, which may improve outcomes.

Prevention
• Vaccination is extremely effective and the best protection.
• Puppies need a series of vaccines every 3–4 weeks until at least 16 weeks old.
• Avoid dog parks or unknown dogs until fully vaccinated.
• Use bleach (1:30 dilution) to disinfect contaminated areas.

ANVH EP-80 Silent killer - tumor rupture 沉默的殺手 - 腫瘤破裂

Over the past few weeks, A Niu has handled three emergency surgeries caused by ruptured tumors — two in the spleen and one in the liver. All three were senior dogs over 10 years old.

Their owners said the same thing: “They were totally fine… then suddenly collapsed.”

When they arrived, the dogs had low blood pressure, pale gums, and signs of internal bleeding.

Ultrasound scans revealed large abdominal masses with active hemorrhage.

These are true life‑or‑death moments. Surgery has to happen
immediately to find the bleeding source and remove the tumor if possible. But not every patient is fortunate enough to make it through such a critical event.

Afterward, many owners ask how they can prevent this from happening. The honest answer is: regular health check‑ups.

Tumors don’t grow big or rupture overnight. By the time symptoms appear, the tumor has usually been developing silently for months.

Prevention will always be better than emergency treatment.

If your dog or cat shows signs like weight loss, a growing belly, pale gums, or sudden collapse, please seek your family vet right away.

阿牛在這幾周內接到了3例腫瘤破裂的緊急手術,2例是脾臟1例是肝臟,都是年>10歲的老狗,主訴都是本來好好的,突然倒下站不起來(血壓低),黏膜蒼白(內出血),所以送來醫院

經檢查後發現腹腔有大的團塊,經超音波確診為腫瘤+內出血

這是和死神拔河的緊急手術,必須馬上開刀找到出血點跟評估腫瘤摘除,但不是每個病患都這麼幸運地可以被搶救回來

術後主人問該怎麼樣可以避免這個狀況? 我想了想只有定期的健康檢查才有辦法預防,腫瘤不會一夕之間長到這麼大或者馬上破裂,要到發生臨床症狀了通常都要至少幾個月以上的時間,只能說預防甚於治療

Today, A Niu would like to introduce a case of Radius and ulna fracture in dogs

🐶 Radius & Ulna Fractures in Dogs are among the most common forelimb fractures, especially in small‑breed dogs. Because the radius is the primary weight‑bearing bone, fractures here significantly affect mobility.

Why These Fractures Happen
1. Small‑breed dogs (toy breeds) Very poor blood supply to the distal radius, making it fragile and slow to heal, so even minor trauma (jumping off furniture, falling from arms) can cause a fracture
2. Larger dogs
Require significant trauma (HBC, falls, gunshot injuries) to fracture the radius/ulna
3. Typical fracture patterns
Both bones usually break together
Distal diaphyseal fractures are especially common in small breeds

Clinical Signs
Sudden non‑weight‑bearing lameness/Swelling of the limb/Pain on palpation/Crepitus or instability (often obvious because of minimal soft tissue coverage)

Diagnosis
Radiographs (2 views minimum) to evaluate fracture type: transverse, oblique, comminuted, open vs closed

Treatment Options
⭐ Surgical Repair (Gold Standard)
Especially critical in toy breeds, where external coaptation has an 80%+ nonunion rate.

ANVH EP-78 Atlantoaxial Luxation(環樞椎脫位)

Today, A Niu would like to introduce a classical case of "AA luxation" in dogs

🐶 AA luxation is instability between the first two cervical vertebrae (C1 = atlas, C2 = axis). This instability allows the axis to shift abnormally, compressing the spinal cord. It is most common in toy and small‑breed dogs, especially Yorkies, Chihuahuas, Pomeranians, Toy/Mini Poodles, and Pekingese.

What Causes AA Luxation?
Congenital (most common in small breeds):
Malformed or absent dens (odontoid process)/Dysplastic or aplastic stabilizing ligaments/Complex craniocervical anomalies
Even mild trauma (jumping off couch, rough play) can trigger clinical signs

Clinical Signs
Signs result from spinal cord compression at C1–C2:
Neck pain (most common and sometimes the only sign)/Tetraparesis or ataxia/Intermittent collapse/Head held low or tilted/Inability to stand or walk
Respiratory paralysis in severe cases (can be fatal)

Diagnosis
Cervical radiographs: increased space between C1–C2, dorsal tipping of axis, dens abnormalities
CT and MRI: excellent for bony detail, dens fractures, congenital anomalies and evaluates ligamentous integrity and spinal cord compression

Treatment Options
Conservative (non‑surgical)Used for: Mild signs/Financial constraints/Cases where surgery is high‑risk
Includes:
Strict crate rest (6–8+ weeks)/External splint or neck brace
/Pain control/Anti‑inflammatory medications

Surgery is recommended for severe cases

🐶環樞椎脫位是指第一跟第二頸椎脫臼,壓迫到脊椎神經造成臨床症狀,常見於小型玩賞犬,像是約克夏,吉娃娃,博美,米你貴賓等品種

什麼會造成環樞椎脫位呢?
通常跟先天發育異常有關,例如椎體發育不全,周邊的韌帶發育不全,加上輕微的創傷就有可能引發(沙發上跳下來,玩得太過劇烈等等)

常見的臨床症狀有:
脖子疼痛,共濟失調(無法控制四肢),歪頭,無法站立,癱瘓,嚴重的case會誘發呼吸抑制

如何診斷?
X-ray是第一步,可以看到C1-C2之間的距離變大,以及椎體的發育異常
但斷層掃描跟核磁共振可以完美的去評估細節以及神經受壓迫的程度

怎麼治療呢?
可以分成保守治療跟手術治療
保守治療用於症狀輕微,有經濟考量或手術風險高的病患-嚴格的限制活動6-8周,疼痛管理,戴頸圈or外固定支架
但如果症狀嚴重像是癱瘓or先天發育異常,手術是唯一的治療方式

ANVH EP-77 Xylitol poison in dogs 狗的木醣醇中毒

Today, A Niu would like to share a case of Xylitol poisoning in dogs

Xylitol is extremely toxic to dogs. Even small amounts can cause life-threatening hypoglycemia (low blood sugar), seizures, liver failure, or death. If a dog ingests xylitol, immediate veterinary care is critical.

Why Xylitol Is Dangerous for Dogs
Sugar alcohol: Xylitol is a sugar substitute found in sugar-free gum, candies, baked goods, peanut butter, toothpaste, mouthwash, and even some medications.
• Insulin surge: In dogs, xylitol triggers a rapid release of insulin, leading to a sudden and profound drop in blood sugar (hypoglycemia) within 30 minutes to 2 hours.
• Liver damage: Larger doses can cause acute liver injury or failure within 24–48 hours.
• Species-specific: Cats, ferrets, and horses are not affected in the same way; dogs are uniquely sensitive.

Symptoms of Xylitol Poisoning: Vomiting/Weakness, lethargy, or collapse/Ataxia (loss of coordination)/Seizures/Coma
In severe cases: jaundice, bleeding disorders, liver failure

Note: The amount of xylitol varies widely between products. Even one piece of sugar-free gum can be dangerous for a small dog.

🩺 What To Do If Your Dog Eats Xylitol
• Do not wait for symptoms. Call your veterinarian or an emergency clinic immediately.

Bottom line: Xylitol ingestion is a medical emergency in dogs. If you suspect your dog ate anything containing xylitol, seek veterinary help immediately — even if they seem fine at first.

如果你的狗吃了木糖醇,請趕快跟你的家庭獸醫聯繫,即使他看起來是很正常的,記住,木糖醇對狗來說是一種劇毒

這個case在吃下口香糖後幾小時內肝指數上升到600多(正常125以下),血糖掉到30幾(正常120-150),住院點滴治療幾天後慢慢恢復正常

ANVH EP-76 Statement related to the false accusation

Recently, A Niu has received several malicious negative comments on Google, accusations such as :

1. A Niu is an “Extortion, retaliation, and zero ethics.” animal hospital.
2.“They refused to discuss the results or the sudden price surge. They demanded $60+ just to answer basic follow-up questions a week later.”
3. “Delete the negative Google reviews or your dog is permanently banned from our clinic.” That’s not healthcare. That’s straight-up blackmail. Etc.

Toward this false accusation, our official response is below:
The owner is lying; what they claim is not true, not even close. We would like to clarify that the appointment was originally a recheck for a pre-existing condition, but new clinical signs were reported by you during the visit.

Because of this, additional blood work was recommended. The estimate was presented, reviewed, and the quotation was obtained before we proceeded. That means the owner definitely knows what we will do and gives us permission to do it. The exam results, comparisons to previous findings, and recommendations for diet and supplements were all explained during that consultation.

No concerns were raised at the time. We only became aware of the owner’s dissatisfaction a week later, following with one-star review related to the $50 recheck fee. Since the report had already been fully explained previously, it is appropriate to charge a recheck fee when using additional doctor time to re-explain the same results—especially to someone who was not present at the original visit.

It looks like many misunderstandings stem from this second-hand interpretation. At A Niu Veterinary Hospital, we do not pressure or threaten clients. All estimates and invoices are reviewed with clients before any treatment or medical behaviour is provided. The statements in the owner’s review significantly misrepresent the events that occurred and have negatively impacted our hospital’s reputation. We will be seeking legal action.

Given the communication barriers and the breakdown of trust, we believe we are no longer the right fit for this owner’s veterinary needs. We are happy to transfer the pet’s medical records to any hospital of owner's choice.

飼主講的並不是事實。我們想澄清，這個主人預約原本是針對既有病況的複診，但在就診過程中提出了新的臨床症狀像是黑便跟多喝水。

因此，楊醫師建議進行額外的血液檢查來排查有無其他問題。檢查所需的費用報價已在進行前呈現、說明並由飼主簽署報價確認後才進行。所有檢查結果、與先前數據的比較，以及飲食與保健品的建議，都已在當次門診中詳細解釋。

門診結束時並未收到飼主的疑慮。我們是在一週後，因飼主針對 50 元加幣複診費留下的一星評論，才得知飼主的不滿。由於報告內容先前已完整說明，若需額外花費醫師時間再次解釋相同結果，尤其是對未參與原始就診的人，收取複診費是合理的。

看來許多誤解源自於這種間接的轉述。在阿牛動物醫院，我們絕對不會，也不可能強迫或威脅飼主。所有預估費用與帳單都會在任何治療或給藥前，與客戶確認並同意後執行。我們絕對是收費公開透明的動物醫院。這個主人的評論內容嚴重扭曲跟背離事實，並對本院的聲譽造成負面影響，我們會採取法律的動作。

鑑於溝通障礙與信任破裂，我們認為已不再適合繼續為這個飼主提供任何醫療服務。我們會將寵物的醫療紀錄做好轉給飼主選擇的任何醫院。

Uroliths, also known as bladder stones, are hard mineral deposits that form in the urinary bladder of dogs. They can range in size from tiny grains to larger stones and are often caused by factors like urinary tract infections, dietary imbalances, or abnormal urine pH.

Common symptoms include:
Blood in the urine.
Straining or difficulty urinating.
Frequent urination in small amounts.
Pain or discomfort in the abdomen.

Treatment options depend on the size and type of the stones. Small stones may pass naturally, while larger ones might require surgical removal or other medical interventions.

臨床上經過影像的檢查可以確認有沒有結石的生成,通常小於0.5cm的特定結石種類可以靠處方飼料溶解或變小,但體積龐大的結石則必須考手術移除

這個病例是一個母狗,近幾周以來頻尿,有時候帶血,經過2周的保守治療不見好轉,和主人討論後決定手術取出
通常公的動物,結石都是小顆但是數量很多,母的都是一大顆結石塞滿膀胱,這個病例是後者

ANVH EP-73 Chylothroax in dogs and cats - 犬貓的乳糜胸

Today A Niu would like to share a rare case of Chylothorax in dogs and cats

🩺 What Is Chylothorax?
• Chylothorax is a type of pleural effusion where chyle—a milky fluid rich in triglycerides—leaks abnormally into the pleural cavity, compressing the lungs and causing breathing difficulties.
• Chyle is formed from dietary fat absorbed in the intestines and transported via the lymphatic system through the thoracic duct. If the duct ruptures or becomes obstructed, chyle can leak into the chest cavity.

Commonly Affected Breeds and Causes
• Dog breeds: Shiba Inu and Afghan Hound are predisposed.
• Cats: Can occur at any age.
Common causes:
• Heart disease
• Thoracic tumors
• Lung lobe torsion
• Diaphragmatic hernia
• Venous thrombosis
• Trauma or surgical injury to the thoracic duct
• Idiopathic chylothorax: No identifiable cause; accounts for most cases.

⚠️ Clinical Signs
• Rapid breathing, coughing
• Lethargy, loss of appetite
• Exercise intolerance, hypoxemia
• Thoracocentesis yields milky or pinkish fluid (if blood is mixed)

Diagnostic Methods
• Thoracocentesis: Fluid analysis for triglyceride concentration (higher than serum) and protein levels.
• Imaging: Chest X-rays, CT lymphangiography, echocardiography.
• Blood tests: To rule out heartworm, infections, electrolyte imbalances.

Treatment Options
• Preferred surgical approach: Thoracoscopic thoracic duct ligation ± pericardiectomy.
• Severe cases: May require cisterna chyli ablation.
• New techniques: ICG fluorescence imaging helps locate the thoracic duct and improves success in minimally invasive surgery.

These breeds often have a genetic predisposition that weakens spinal discs over time.

Types of IVDD
There are two main types:
• Hansen Type I: Sudden rupture of the disc, often seen in chondrodystrophic breeds (short-legged dogs). The disc material becomes calcified and can shoot out, causing acute pain or paralysis.
• Hansen Type II: Gradual degeneration, more common in larger breeds like German Shepherds. It causes chronic pain and spinal cord compression over months or years.
A third type may result from trauma or strenuous activity, causing spinal bruising without long-term compression.

Symptoms to Watch For
• Limping or dragging limbs
• Hunched back or neck stiffness
• Reluctance to move or jump
• Loss of coordination or balance
• Sudden paralysis

If you notice these signs, seek veterinary care immediately to prevent permanent damage.

Diagnosis & Treatment
• Diagnosis: Typically involves a neurological exam, X-rays, or advanced imaging like MRI.
• Treatment Options:
• Medical: Anti-inflammatory drugs, pain relief, crate rest
• Surgical: Required for severe cases with spinal cord compression
• Rehabilitation: Physical therapy, acupuncture, hydrotherapy

Prevention & Home Care
• Keep your dog at a healthy weight to reduce spinal strain
• Avoid jumping from high places (use ramps or stairs)
• Provide supportive bedding
• Monitor for early signs and act quickly

For dogs with mild IVDD, home care and lifestyle adjustments can make a big difference.

今天阿牛要介紹的是狗的椎間盤疾病
椎間盤疾病是指脊椎方面的問題造成疼痛,移動障礙,嚴重的case甚至癱瘓,特別是在一些特殊的犬種

椎間盤疾病通常是因為在脊椎骨中間的緩衝組織(椎間盤)發生腫脹,破損而壓迫到神經,進而引發的症狀,神經方面通常會引起較為劇烈的疼痛跟癱瘓,排便尿也會受到影響(神經控制)

好發犬種為:臘腸,柯基,西施,法國/英國鬥牛犬,巴吉度獵犬等
這些犬種因為基因上的缺陷,而特別容易出現這些問題,其中25
%的臘腸狗在一輩子狗生當中一定發生過IVDD

IVDD有分成三種類型:
1. Hansen type I:椎間盤突然的破損,常見於短腿體型的狗,椎間盤物質溢出且壓迫到神經,造成急性且劇烈的疼痛,有可能伴隨癱瘓或排便尿困難
2. Hansen type II: 椎間盤的退化腫脹造成神經的壓迫,較常發生於大型犬種,引起慢性疼痛,這問題通常伴隨著數月到數年的時間
3. 創傷造成,專間盤暫時性的水腫/瘀血造成壓迫,經過治療後恢復正常,通常不會留下後遺症

臨床症狀有:跛行,拱背,脖子僵硬,無法移動,劇烈的疼痛,癱瘓等
常見的檢查有理學檢查跟X光,想要確診部分必須做高階的影像學檢查

那我們怎麼治療?
口服消炎止痛藥,限制活動強迫休息,嚴重的case必須進行手術,接著是漫長的復健之路

這個疾病預防勝於治療,盡量避免垂直的運動,保持健康的體重,睡的床要有適當的支持,以及早期就醫
