To every woman with a chief complaint of shoulder pain. I’m sorry.
I gave you pain meds. I put your arm in a sling. I told you to follow up with ortho. I did what I was trained to do.
But I never asked if your pain was cyclical.
I never asked what else had changed in the last year.
I didn’t connect your shoulder to your hormones because nobody taught me that estrogen receptors are EVERYWHERE including in your joint tissue.
You were probably 42, maybe 45. You told me it came out of nowhere. One day you woke up and couldn’t lift your arm. The orthopedic surgeon likely said “this just happens to women your age” and sent you to physical therapy.
Here is what we both missed:
Estrogen is anti-inflammatory.
When it drops during perimenopause, inflammation rises everywhere those receptors live. Your rotator cuff included.
Your shoulder wasn’t failing you randomly. It was responding to a predictable hormonal shift that neither of us were trained to recognize.
I know better now. Every woman in my practice and in the ER gets asked: “Is this cyclical? What else has shifted?” Because your joints aren’t separate from your ovaries.
Nothing in your body is.
