• Your doctor fills out a requisition, the official form that explains what exam you need and why.
• They’ll either fax it to the imaging provider or hand it to you so you can book your appointment.
• And the best part? Even if your doctor gave you a requisition for another provider, you can still use it at any of our 15 clinics across Calgary and area. Use the clinic that’s most convenient for you!

🦴 Fun fact: The phrase “rotator cuff” wasn’t widely used until the 20th century—before then, shoulder tendons were often lumped under generic “shoulder sprain/strain”.
