The tongue is a direct mirror of what is happening inside the body — especially the organs, blood, fluids, and overall balance of energy (Qi).
In TCM, different areas of the tongue are linked to different organs and changes in colour, shape, or coating can hint at which systems are out of balance.
Unlike pain or fatigue, which are subjective, the tongue gives more objective clues.
These are the things we check when we observe patient’s tongue:
Colour - pale, pink, red, or purple
Coating - thick, thin, greasy, absent
Shape - swollen, thin, cracked, teeth marks
Location - tip, centre, sides, back/root
The tongue doesn’t change instantly, so it is especially useful for understanding chronic or ongoing imbalances, not just short-term issues. It is also a way for practitioners to track progress of the treatment. For example, improvements in colour or coating can suggest that treatment is working.
Tip of the tongue reflects the heart. The heart governs the mind and emotions, so changes here often relate to mental and emotional states.
Front sides (just behind the tip) are lungs. This area connects to breathing, immunity and skin health.
Centre or the tongue reflects the spleen and stomach. It reflects digestion, energy production and fluid metabolism. Digestive issues often show up here before symptoms feel serious.
Sides of the tongue show liver and gallbladder condition. The liver is linked to emotional flow, so this area often reflects stress, frustration, or tension.
Back or root of the tongue - kidneys, bladder and (small/large) intestines. This area reflects long-term vitality, hormones, and aging in TCM theory.
