As the year 2025 turns, I look back across the twin landscapes of my life—the intimate world of my clinic and the vast arena of public trust.

Thousands upon thousands of encounters in my clinic. Each a universe. A chart of symptoms, yes, but more profoundly, a record of human vulnerability and resilience. A baby’s distraught cry, the chronic pain engraved into a face, the silent scream of anxiety, the relief of a managed condition. These encounters were my education in the raw truth of life, in the social determinants that script our health more than genetics ever could.

The determinants piled high: poverty, loneliness, fear, inequality—the real pathogens. The fuel was hours stretched thin, sleep sacrificed, the constant hum of burnout a background noise to the symphony of need.

The cost was measured in fatigue, in sleepless nights, in the weight of carrying so many stories. Yet, from this transformative journey, a positive balance emerges. Not in a ledger of cures, but in the subtle shifts: the personal reassurance that changed a family’s course, the early detection that saved a community member, the hope whispered that took root. A tangible sense of impact on individuals, families, and the fabric of our community.

That hard-earned trust became the foundation for a new, daunting chapter.

More than 16,000 votes in the Federal Election of 2025. A number comparable to the number of greetings at the clinic, but now from the public square. The consulting room expanded to the town hall, the diagnosis to a policy platform, the treatment plan to a vision for systemic change. This trust, given at the ballot box, is a profound extension of the covenant I knew in medicine. It is a mandate to heal not just one patient at a time, but the very systems that make them sick.

A major win? Yes, for a beginner. A victory? Not yet. Victory is when the vision becomes reality. This new path is tougher, more complex, and more necessary than ever. The need for visionary change in how we care for one another is urgent. The trust of thousands upon thousands of patients and over 16,000 voters is my compass and my charge. It demands harder work, deeper connections, and an unwavering commitment to translate grassroots truth into political will.

And through this whirlwind, my center holds: a happy family life. My children, blossoming in their chosen paths. My partner, my better half—the steadfast anchor who grounds me, supports me, and keeps my focus clear amidst the storm. This private joy is the bedrock of all public service.

So, this is a convergence. A new beginning where lived experience meets public duty.

2026 calls. It calls for the healer’s heart and the advocate’s courage. It carries the weight of two profound trusts—earned at the bedside and at the ballot box. It is fueled by the love of family and the stories of thousands.

With a heart full of gratitude and a spirit ready for the work ahead, I step forward.

URGENT: A New Pathway for SOME Doctors, But What About the IMGs Already Here?

IRCC just announced a new Express Entry category launching in early 2026. It’s for international doctors already working in Canada on a temporary basis, with at least one year of Canadian work experience. The goal is to give them a "clear pathway to permanent residence" to fill critical health workforce gaps.

On the surface, this sounds like a positive step. We absolutely need to retain skilled doctors contributing to our healthcare system.

BUT THIS ANNOUNCEMENT HAS A GLARING, PAINFUL OMISSION.

It completely overlooks International Medical Graduates (IMGs) who are ALREADY Permanent Residents or Canadian Citizens.

Think about that. We have thousands of doctors already living here permanently. They pay Canadian taxes. They are raising their kids in Canada. They have invested everything into building a life here.

Many have:
✔ Completed the long, costly, and rigorous credential verification process.
✔ Passed all required medical licensing exams (NAC OSCE, MCCQE).
✔ Completed necessary assessments and residencies (where spots are available).
✔ They are ready, willing, and desperately needed to practice.

Yet, they remain stuck in a systemic limbo, unable to get licensed or find a residency position due to a severe shortage of spots and systemic barriers within provincial licensing and healthcare systems.

The IRONIC INJUSTICE:

Canada is creating a new immigration stream for temporary resident doctors (a good thing), while simultaneously failing to utilize the immense, fully-qualified doctor talent pool that is already permanently settled here. We are importing solutions while ignoring the ones already in our house.

This isn't about pitting one group against another. All internationally-trained doctors are needed. But a coherent national health workforce strategy MUST prioritize unlocking the potential of those who have already proven their commitment to Canada by making it their permanent home.

We need to demand MORE than this.

1. For IRCC & The Federal Government: Partner with provinces to tie immigration pathways to concrete commitments to increase residency spots and streamline licensing for all IMGs, especially PRs and Citizens.
2. For Provincial Governments: This is where the bottleneck is. We need MASSIVE, IMMEDIATE INVESTMENT to create more residency positions for IMGs. The federal government can help fund this.
3. A Unified Message: Retention of temporary workers AND utilization of permanent residents are two sides of the same coin. We need a strategy that addresses both with equal urgency.

This new category is a small step on one path, but the main roadblock remains. Our healthcare system is crumbling. Patients are suffering. And we have a solution—thousands of doctors—already here, living next door, waiting to serve.

It’s time for a real, comprehensive plan.

“Jail Not Bail: A Common-Sense Approach to Community Safety” *বাংলায় পড়ুন শেষে*

An Enlightened Afternoon Town Hall meeting with MPs: Arpan Khanna, Larry Brock, Rhonda Kirkland, Jamil Jivani

It’s an honour to be present in the town hall meeting today in Ajax to discuss one of the most critical issues facing our community: community safety.

I come to this town hall not just as a conservative MP candidate on record of Scarborough Southwest, but as a family physician. My career has been dedicated to diagnosing problems and prescribing treatments that heal. And today, I must give you my professional diagnosis: Our system is sick, and it's making our community sick.

(The Physician's Diagnosis)

"As a doctor, my first duty is to 'do no harm.' But our current catch-and-release bail system is actively harming our community's health. We see the symptoms every day. The Liberal’s soft-on-crime experiments have failed. Let me be clear: as a physician, I wouldn't recommend release a contagious patient into a healthy nursery, and I will not support a system that releases dangerous, repeat, and violent offenders into our healthy community."

"So, what is the cure? It starts with a principle we all understand: accountability. Conservatives believe in personal responsibility and the rule of law. A system that allows dangerous individuals back into our communities with no immediate consequence undermines both.

Let's be clear: Bail was never intended to be a revolving door for repeat, violent offenders. Its purpose is to ensure a defendant appears for trial. When someone poses a clear danger to the public, they should be held. It's that simple.

And this isn't about incarcerating non-violent, low-level offenders. This is about using common sense. A 'Jail Not Bail' policy for violent and repeat offenders protects the public and sends a clear message: in Canada, crime must have consequences."

"My medical career has been dedicated to protecting life and promoting strong, healthy families. But how can families thrive when they don't feel safe in their own neighborhoods?

The current system abandons crime victims, often from our most vulnerable populations. It prioritizes the rights of the accused over the safety of the victim. A true pro-family policy puts the safety of law-abiding citizens first.

That’s why keeping violent predators behind bars pre-trial is a pro-life position. It protects the innocent lives of potential future victims.

We believe in justice for victims, safety for our families, and treatment with accountability for those who are ill. Our 'Jail Not Bail' policy delivers on all three. This isn't about being tough on crime; it's about being smart on crime. It's common-sense conservatism applied to public safety."

"Finally, as a conservative, I believe in fiscal responsibility. So we have to ask: what is the cost of a burglary? A car theft? A life lost? The financial and human cost of revolving-door crime far outweighs the cost of secure detention for dangerous individuals.

Ultimately, a safe community is a healthy community. It's where businesses open, jobs are created, children can play outside, and parents and families can sleep at night with peace. Restoring public safety isn't just a justice issue—it is the best economic development tool we have.

Together, with this common-sense approach, we can heal our system and restore safety to the streets of Scarborough Southwest.

· "একটি নিরাপদ community হল একটি সুস্থ community। সেখানেই ব্যবসা-বাণিজ্য চালু হয়, চাকরি সৃষ্টি হয়, শিশুরা বাইরে খেলতে পারে এবং পরিবার নিশ্চিন্তে ঘুমাতে পারে। জননিরাপত্তা পুনরুদ্ধার করা আমাদের কাছে অর্থনৈতিক উন্নয়নের সেরা হাতিয়ার।"

Together, with this common-sense approach, আমরা আমাদের ব্যবস্থাকে সুস্থ করতে পারি এবং স্কারবোরো সাউথওয়েস্টের রাস্তায় নিরাপত্তা ফিরিয়ে আনতে পারি।

“An Open Letter Against Ontario's Exclusionary CaRMS Residency Policy”

I write to you today with grave concern regarding the recently announced changes to the CaRMS 2026 R-1 Match requirements. The decision to restrict eligibility to CaRMS R-1 applicants who have attended an Ontario high school effectively slams the door on qualified International Medical Graduates who have immigrated to Canada and are Canadian citizens or permanent residents. This is not merely a policy adjustment—it is a systematic exclusion of dedicated healthcare professionals who have chosen Ontario as their home.

This abrupt, mid-process implementation demonstrates a profound disregard for physicians who have invested years working toward Canadian licensure. These individuals have undertaken rigorous examinations, navigated complex credentialing procedures, and made significant personal sacrifices—all based on a belief in Ontario's commitment to fair and transparent processes. Many have established roots in our communities, only to discover their contributions are no longer welcome.

This exclusionary policy arrives amid an unprecedented healthcare crisis:

# The crisis is now: As of July 2024, 2.5 million people in Ontario lack a family doctor—a surge of over 160,000 in just six months (Ontario College of Family Physicians).
# The problem is national: An estimated 6.5 million Canadian adults (22%) do not have a family doctor or nurse practitioner.
# The collapse is imminent: Ontario needs 3,500 family doctors immediately, while nearly 40% of current practitioners near retirement.

While our healthcare system strains under these pressures, the government has chosen to implement barriers that prevent qualified, ready-to-serve physicians from contributing to the solution. This decision not only contradicts Ontario's stated values of inclusion and diversity but actively undermines our collective efforts to address the healthcare crisis.

We therefore demand the Ontario government:

1. Immediately reverse the high school requirement and honor the original eligibility criteria for all 2026 CaRMS applicants.
2. Create a dedicated IMG Stream to ensure equitable access for immigrant physicians who have chosen Canada as their home.

The solution to our healthcare crisis stands ready to serve. We urge you to remove these arbitrary barriers and allow dedicated physicians to care for Ontario's patients.
