Carpal Tunnel Syndrome #3

Carpal tunnel syndrome is a compressive neuropathy of the median nerve as it passes through the wrist into the hand.

Typical symptoms include:

-Aching or burning pain on the palm side of the wrist
-Numbness or tingling in the thumb, index, middle, and half of the ring finger (first 3½ fingers)

Symptoms are often worse at night, leading people to wake up with numb fingers and try to "shake out" their hand to relieve the discomfort
If the entire hand or arm is numb, the compression may be higher up the arm — and in some cases, more than one nerve can be compressed at multiple sites.

The median nerve can become compressed due to:

-Thickening or scarring of the transverse carpal ligament
-Swelling of the wrist flexor tendons

Common risk groups include:

-Pregnant patients (due to fluid retention)
-People with metabolic conditions like diabetes or obesity
-Manual labourers, machine operators
-Desk workers or anyone doing repetitive wrist/keyboard tasks

Diagnosis is often clinical — based on:

-Risk factors
-Pattern of symptoms
-What makes it better or worse
In fact, a good history and physical exam can be almost as accurate as a nerve conduction study!

A nerve study is typically used if:
The diagnosis is unclear, or
Surgery is being considered

Carpal Tunnel Syndrome #2

Most people know that Carpal Tunnel Syndrome is 'numbness tingling or pain in the wrist / hand and fingers. But, did you know this diagnosis has a very specific symptom map? For example it doesn't affect every finger, and certain hand positions should provoke the pain while others should relieve it.

Just have a look at a few of these symptom diagrams and how many of them end up with symptoms in the hand and fingers. (NONE OF THESE ARE CARPAL TUNNEL) Highlighted are trigger points or 'knots' in the pecs, muscles in the neck and muscles in the forearm. Pinched nerves in the neck, armpit region, and pinched nerves in the elbow. It is often that even when a nerve isn't involved, trigger points will refer a sensation of tingling making it confusing as to where the problem is coming from.

Next we'll highlight what makes the symptoms of carpal tunnel specific and how we can diagnose it with a clinical exam.

Have you been told you have carpal tunnel syndrome—but your symptoms don’t quite match what you expected?

Recently, three patients came to me with a diagnosis of carpal tunnel syndrome—but their symptoms didn’t line up with classic signs of the condition

I’m launching a mini-series on carpal tunnel syndrome: what it is, how to recognize it, and what you can do to manage it. Stay tuned!

Other conditions that may be mistaken for carpal tunnel syndrome:

- Pronator teres syndrome / anterior interosseous nerve syndrome – nerve compression in the forearm

- Cubital tunnel / Guyon’s canal syndrome – nerve compression at the elbow or wrist

- Cervical radiculopathy / thoracic outlet syndrome – neck/shoulder nerve compression

- Diabetic neuropathy / B12 deficiency – systemic nerve issues

- Wrist osteoarthritis / ganglion cysts – structural problems causing pain

- Mechanical wrist pain - tendon and joint-related pain

- Referred pain from trigger points – pain originating in neck, shoulder, or arm

- De Quervain’s tenosynovitis – inflammation of tendons on the thumb side of the wrist, causing pain when gripping or twisting.

- Intersection syndrome – inflammation where two sets of wrist tendons cross, causing pain and swelling in the forearm.
