EPISODIC
🩺Tailored assessment---One concern per visit---$55-15min/$70-20min/$85-25min

- Urinary symptoms (pain, blood, frequency, incontinence, dribbling, etc.)
- Hearing problem, congestion, sinus pain
- Skin (lesion, rash, acne, rosacea, cold sores, bites, fungal toenail, diaper rash)
- Women's Health (vaginal irritation/dryness, contraception, pregnancy concern, peri-menopause/menopause symptoms, cancer prevention/screening)
- STI concern/testing
- Musculoskeletal pain or stiffness (back, hip, knee, small joints, gout, arthritis)
- Gastrointestinal (blood in stool, change in bowel pattern, constipation, weight loss, anemia, hernia, GERD)
- Cardiovascular (hypertension, high cholesterol, cough, mild shortness of breath, leg swelling, palpitations)
- Respiratory (asthma, allergies, COPD)
- Sleep problem (snoring, waking gasping for air, daytime drowsiness/dosing off)
- Immunization assessment, prescription (ie. Shingles, RSV)
- Medication Refill (no benzodiazepines, narcotics)
- Sick note (when appropriate)
- Referral to specialist or community program (when appropriate)
