Stonebridge Community Services provides Home Care & Wellness to individuals in the community and surrounding area. Stonebridge Community Services is a not for profit, organization providing in-home health care and preventative health services designed to help people maintain good health and independence through the aging process. Stonebridge services are available to everyone in the community and are designed to enhance or support a wide range of health services. Home Support Services provides a holistic approach to care, where we respect the dignity and personhood of the client and their family. Each and every day Stonebridge Community Services staff deliver a comprehensive suite of personal and family centered health care services, in client’s homes, with compassion and professionalism respecting dignity of person and promoting self actualization. All home support services are supervised by a registered nurse. Preventative Health Services - Research and experience continues to show that regular exercise and activity are a vital part of a healthy lifestyle. Our Health & Wellness Services offers several unique programs to help keep you active all year round. Our programs are geared to all age groups and to all different fitness levels. Transportation and Rides - We support independence by providing safe, reliable and low cost transportation services. Whether you need a ride to attend a medical appointment, want to go shopping or just remain active in the community, we can help. We travel throughout the South West Region. Wheel chair accessible vehicles are also available. Stonebridge Health & Wellness offers FREE group exercise classes to seniors. These classes are funded by the Hamilton Niagara Haldimand Brant Local Health Integration Network and our VON SMART classes are funded by The South-West Local Health Integration Network.

🦶 FALLS PREVENTION 🦶 let’s talk about our feet! Having feet that are healthy and pain free means easier walking, which means a reduction in our falls risk. It’s really important to check your feet for blisters, wounds, ingrown nails, calluses on a daily basis. If you live with diabetes or poor circulation, foot care is especially important and should be a daily priority. When looking for shoes, fit is really important and I do recommend that you get your feet measured as our feet can grow as we age. 👟 comfort and support are key!

⚠️ FALLS PREVENTION ⚠️ this week, I’m demonstrating how to safely get up from a fall. This is a technique you can use if you were alone - I think that we can safely assume that if you fell in a crowded place, somebody would be there to help you ☺️. To start with, it’s important to take your time. Of course if the place you fell puts you in immediate danger we want to move quickly, but otherwise take your time. Take some deep breaths, assess any injuries. You need to slowly make your way to your feet using whatever is around you to help. For example, if you fell in the driveway, you could crawl over to your car and use that for support, if you fell in the house using the stairs or other furniture can also help. I encourage everybody to test this out today and see how easy it is for you to get up and down off of the floor. Try it out at home and if it’s difficult, don’t be afraid to practice!

A festive flashback from last year! 🎄

Oh, what fun it was to ride on the MSC & Stonebridge float!

Our team had a blast spreading holiday cheer at the Tillsonburg Santa Claus Parade. The chilly night was no match for the warmth of the community.

Thanks to everyone who came out to wave, cheer, and celebrate the season with us — such a special memory! 🎅
