Care isn’t only what we provide, it’s how we show up.

In the kitchen at Journey Home Hospice | Saint Elizabeth Foundation, our volunteers offer more than meals. Through preparing and sharing food, they help create moments of connection, comfort, and belonging—supporting dignity and care at every stage.

These everyday acts carry extraordinary meaning, reminding us that compassion is often found in the simplest gestures.

Thank you to those who give their time and heart to this work.

Day 2 at the Canadian Association for Spiritual Care/Association canadienne de soins spirituels conference.

Saint Elizabeth Foundation was honoured to open the day with a Spark Talk led by our Executive Director, Fr. Dr. Matthew Durham, sharing our work with the full conference delegation. It was a powerful moment and a meaningful opportunity to engage with so many individuals committed to advancing compassionate care.

We are grateful for our partnership with CASC. Opportunities like this reflect what is possible when we come together around a shared purpose and vision.

As the conference continues, we remain committed to advancing the field of spiritual health through practice, research, and a deepening focus on equity, both nationally and internationally.

Looking ahead, we are encouraged by the strength of collective leadership and shared commitment shaping the future of this work.

Yesterday marked Day 1 at the CASC/ACSS National Conference in Ottawa.

Saint Elizabeth Foundation is proud to be present as the Platinum Sponsor, joining colleagues from across the country convened by the Canadian Association for Spiritual Care/Association canadienne de soins spirituels. We are grateful for the warm welcome and for holding such an important space advancing spiritual care in Canada.

A meaningful moment from Day 1 included our Executive Director, Fr. Dr. Matthew Durham, alongside the visionary CEO of CASC, Jeffrey Chant (pictured), reflecting on the strength of leadership and community within this field.

The theme, “Thresholds of Transformation: Spiritual Health in the Age of Innovation,” is already prompting thoughtful dialogue. What is evident is that spiritual care is not simply adapting to change, but helping to shape how care is understood in the context of complexity, technology, and evolving human need.

The day created space to begin conversations around the work of Saint Elizabeth Foundation including the newly launched Centre for Equity and Innovation in End-of-Life Care, alongside our national spiritual health, grief, and bereavement services. These conversations are resonating, particularly around equity, access, and the importance of community-informed approaches at the end of life.

More to come as the conference continues.

This visit from our dedicated volunteer Marney and an Elizz Paws therapy dog, Rosa, offered our residents a peaceful moment of connection, reminding us how deeply support can be felt beyond words.

At Saint Elizabeth Foundation | Journey Home Hospice, care is expressed not only through clinical expertise, but through presence, companionship, and the small gestures that help people feel at home.

We’re grateful for our volunteers and the Elizz Paws therapy dog's for bringing warmth, comfort, and connection to our residents and brightening their day.

Through the Saint Elizabeth Foundation’s National Centre for Equity and Innovation in End-of-Life Care, our work is rooted in a clear, community-driven approach: listening deeply, honouring each person’s dignity and lived experience, empowering those who provide and receive care, and advancing equity through learning and collaboration.

Together with partners across Canada, we are working to expand access to compassionate, high-quality end-of-life care for individuals and communities who are too often underserved.

Supporting healthier communities means recognizing the deep connection between environment, health, and care.

At Saint Elizabeth Foundation | Journey Home Hospice, we see how these factors shape the well-being of the individuals and communities we serve, especially those facing vulnerability.

This Earth Day, we’re proud to stand alongside SE Health in advancing more sustainable, compassionate, and community-driven approaches to care.

Journey Home Hospice is grateful to be featured in the April edition of Catholic Charities of the Archdiocese of Toronto Insights & Encounters newsletter.

Our story reflects a season of renewal while marking nine years of Journey Home Hospice—highlighting dignified, person-centred care for those experiencing homelessness and housing vulnerability.

It also speaks to the importance of meeting individuals where they are, honouring identity, restoring connection, and advancing more compassionate systems of care.

We’re honoured to share this reflection and the impact of this ongoing work.

Once upon a time, these were just moments—snapshots of who we were, without knowing where life would lead.

Today, those stories have led us here—becoming the people behind the care.

At Saint Elizabeth Foundation | Journey Home Hospice, our team brings compassion, dignity, and humanity to every moment of the end-of-life journey.

This is what care looks like: a community of people coming together to create connection, comfort, and support when it matters most.

This is who we are, then and now 🤍
