In the spirit of our namesake, Saint Elizabeth, and her dedication to helping the sick, poor, and hungry, Saint Elizabeth Foundation | Journey Home Hospice remains committed to this type of social impact to this day – especially as we mark her feast day today, Nov. 17.

As we once again take time to reflect on over 115 years of service at SE Health, we not only honour the legacy of care established by our founders, but also recognize the leadership and innovation that continue to define us today. The vision of the four nurses who started Saint Elizabeth inspires us to keep leading with courage and creativity as we bring Hope and Happiness to those we serve.

On Feast Day, we come together not only to remember this extraordinary legacy, but also to ask: How we can lead change in our own communities.

Simple, tangible gestures can spark deeper connection, build trust, and create belonging for those who may feel forgotten in every community. We can each take Saint Elizabeth’s example of giving back in our daily lives and treat others with respect, compassion, and kindness.

EXCITING NEWS! 📢

Saint Elizabeth Foundation and Pallium Canada are proud to announce a new national partnership to expand access to equitable palliative care across the country.

This partnership represents an important step toward the long-term sustainability of palliative care education throughout the country. It will support continued advocacy and the delivery of programs that promote compassionate care for structurally vulnerable populations, including individuals facing homelessness, mental health challenges, and substance use.

This initiative builds on Saint Elizabeth Foundation’s launch of a National Centre for Equity and Innovation in End-of-Life Care in June, and Pallium Canada’s ongoing efforts to expand national palliative care education. Together, our organizations aim to create a joint national model for palliative care learning that is tailored to the needs of individuals who are often excluded from traditional healthcare systems.

Have you ever wondered what motivates someone to volunteer at Journey Home Hospice?

We're inviting our Saint Elizabeth Foundation | Journey Home Hospice volunteers to share why it’s so rewarding to be part of a team that cares for people who are homeless and at end-of-life.

Jenny loves interacting with our patients and is an expert at planning fun, themed holiday celebrations. Her recent “Tropical New Year’s Eve” party was a huge hit!

Across Canada, our dedicated team provides compassionate, holistic support for patients, families, and care partners, helping them navigate life’s most profound moments with dignity, understanding, and hope.

Every day, more than 9,000 members of SE Health’s care team deliver over 25,000 care visits, ensuring every person is seen, heard, and cared for—body, mind, and spirit.

Under the exceptional leadership of Cindy (Cynthia) M. Dwyer, MA, BCC, our national spiritual care and bereavement program continues to expand its reach and deepen its impact, reminding us all that healing happens in connection and relationship.

Our team was proud to attend the 46th Annual Cardinal’s Dinner, hosted by the Archdiocese of Toronto at the Pearson Convention Center in Brampton.

At Saint Elizabeth Foundation | SE Health, we are deeply grateful for the Archdiocese’s ongoing support of Journey Home Hospice, our downtown Toronto hospice that provides compassionate, dignified end-of-life care for people experiencing homelessness and vulnerability — which has also been the impetus for our newly launched Saint Elizabeth Foundation National Centre for Equity and Innovation in End-of-Life Care.

Events like the annual Cardinal’s Dinner bring together leaders from across faith, community, and service, a true reflection of the spirit of collaboration that strengthens our shared mission to bring hope and healing where it’s needed most.

Members of our Saint Elizabeth Foundation team, Journey Home Hospice executive director Fr. Dr. Matthew Durham, and director of care Felicia Kontopidis, were honoured to join colleagues at Hospice Palliative Care Ontario MPP Day 2025 at Queen’s Park this afternoon.

This year proudly celebrate 50 years of hospice palliative care in Canada, a movement rooted in dignity, compassion, and ensuring people can live well until the end of life.

The theme for the day:
Hospice Palliative Care = Quality of Life. Living Well Until End of Life.

It was wonderful to have representation from Saint Elizabeth Foundation that highlighted our work at Journey Home Hospice, and our specialized hospice model for people experiencing homelessness and structural vulnerability.

Our team also shared updates from our newly launched Saint Elizabeth Foundation National Centre for Equity & Innovation in End-of-Life Care, which is advancing equitable, trauma-informed, and culturally safer approaches across Canada.

They were able to meet with Members of Provincial Parliament from all parties to highlight the essential role of:
✨ Hospice residences & specialized hospice care
✨ In-home hospice support
✨ Grief & bereavement services
✨ Community leadership, volunteers & donors

Hospice care matters because it:
✅ Helps end hallway medicine by reducing hospital use
✅ Supports people to remain at home or in a home-like hospice
✅ Responds to emotional, practical & spiritual needs
✅ Protects the wellbeing of families & caregivers
✅ Delivers compassionate, person-centred care
✅ Is one of Ontario’s most efficient and impactful care models

As we collectively mark five decades of hospice leadership, we recommit to building a future where every person, including those facing homelessness, trauma, or systemic marginalization, has access to compassionate, high-quality end-of-life care.

Equitable care can be viewed through many different lenses, but the same truth remains - all people, regardless of their circumstance, deserve to be cared for with dignity and compassion.

The past three days have been a wonderful opportunity to network and collaborate to shape the future for Canada's most vulnerable citizens.

It’s hard to believe the final day of the Canadian Alliance to End Homelessness National Conference is already here. This week has been filled with inspiration, learning, and heartfelt conversations about what it truly takes to build communities of care.

Today’s fireside chat with Federal Minister Gregor Robertson and CAEH President & CEO Tim Richter reminds us how collaboration and shared purpose can shape the future of housing and health in Canada.

As participants gathered for the networking breakfast to connect around themes like faith-based care, Indigenous and youth homelessness, and lived experience, we were reminded that real change begins in dialogue, through listening, empathy, and action.

At Saint Elizabeth Foundation, we remain deeply committed to advancing compassion and dignity for all, ensuring that every person has a safe place to call home, and the care and belonging that make it one.

Because every life matters, to the very last moment.
