Moments of calm, creativity, and connection 🌿

Patients at Journey Home Hospice Toronto enjoyed a cozy wellness activity, wreath making that filled the space with seasonal joy, connection, and comfort.

Wellness activities like this are part of our holistic model of care, allowing our residents to have individualized care plans that honour their life’s journey. These complimentary wellness activities work in conjunction with care that addresses pain and symptom management, psychosocial needs, and spiritual care, offered in a flexible, responsive, and respectful setting.

This week, through Saint Elizabeth Foundation's annual Hope & Happiness Campaign, our incredible Foundation Ambassadors from the Erie St. Clair service delivery centre in Windsor are once again sharing the warmth. Items collected through their SDC collection drive, along with this year’s campaign blankets, are being distributed through the Giving Fence, a simple yet powerful way for individuals to take what they need, when they need it.

This year, the Giving Fence is set up at the The Salvation Army Windsor Centre of Hope, featuring coats, scarves, gloves, and beautiful quilts. The Ambassadors were also able to make a donation to the Welcome Centre Shelter for Women & Families

This collaboration brings together shared learning from Journey Home Hospice and the Canadian Centre for Equity and Innovation in End-of-Life Care, with a collective focus on reaching people who are too often excluded from end-of-life care due to poverty, homelessness, or systemic barriers.

By combining data, advocacy, and practical innovation, this partnership is grounded in a simple but powerful goal: ensuring everyone can access compassionate, dignified care when they need it most—no matter their circumstances. It’s hard not to think that Dame Cicely Saunders would be deeply proud to see her legacy carried forward through work so firmly rooted in humanity, equity, and justice.

Saint Elizabeth Foundation | SE Health had the extraordinary opportunity to travel across the pond to London, Liverpool, and Dublin, connecting with remarkable colleagues and learning from the innovative work happening across the UK and Ireland.

These conversations reaffirmed something we hold deeply at Saint Elizabeth Foundation | SE Health: compassion has no borders, and the pursuit of equity without borders must be shared across nations.

One of the most meaningful moments was announcing the Saint Elizabeth Foundation’s National Centre for Equity and Innovation in End of Life Care and our new partnership with Hospice UK. This international collaboration strengthens our collective efforts to improve access, share knowledge, and advance solutions for those most often forgotten by traditional systems.

We were honoured to present at the National Hospice UK Conference, sharing the Journey Home Hospice model and Canada’s experience delivering specialized palliative care to people who face homelessness, poverty, and structural inequities. The response was humbling, and a powerful reminder that the Canadian challenges we are tackling resonate globally.

And while this visit inspired us, it also deepened our commitment to the urgent needs at home. Canada’s communities continue to face significant gaps in end-of-life care, and the lessons learned abroad will fuel our next steps here, ensuring that innovation, compassion, and equity continue to guide our work.

This experience reaffirmed our shared belief across borders:
Every life matters to the very last moment.

✨ This week in Hope & Happiness Holiday News… ✨

Saint Elizabeth Foundation | Journey Home Hospice is grateful to receive a meaningful contribution from the Burden Bear Ladies. Known for sewing comforting bears for patients, they added a special seasonal touch by collecting monetary donations, cozy PJs and loungewear, and creating hand-made fleece blankets for our patients. Their kindness brings comfort and warmth into the season.

We are honoured that the Canadian Hospice Palliative Care Association (CHPCA) has featured the Saint Elizabeth Foundation National Centre for Equity & Innovation in End-of-Life Care in their recent blog post.

The Centre was created to advance equitable, culturally grounded, and community-led approaches to end-of-life care — guided by four core pillars:

1. Listen to stories, needs, and communities.
2. Honour dignity, traditions, and humanity.
3. Advance equity and innovation in end-of-life care.
4. Empower caregivers, communities, and systems to do better.

Nancy Lefebre and Fr. Dr. Matthew Durham had the pleasure of visiting Fintan Fagan, Chief Executive Officer of St. Francis Hospice Dublin, on behalf of Saint Elizabeth Foundation | SE Health, and were left deeply inspired.

They were met with extraordinary hospitality and an openness to share the remarkable work they are doing at the hospice. Their holistic, patient- and family-centred approach to hospice care is truly exceptional. It is clear that every member of the St. Francis Hospice Dublin team is committed to dignity, compassion, and thoughtful innovation in how they support patients and families.

We are grateful for the time, insights, and generosity extended to our team during their visit.

We look forward to continuing the conversation, and to exploring opportunities for international collaboration and shared learning as we work together toward better care for all.

Today’s themes of fundraising sustainability, workforce wellbeing, and leadership rooted in empathy resonate strongly with the work happening across Canadian communities. This partnership with Hospice UK continues to give us valuable insights that strengthen our efforts to support equitable and resilient hospice and palliative care at home.

🌟 At Last, Today the Announcement Became Reality …

A Global Turning Point for Equitable End-of-Life Care: Our New Partnership with Hospice UK.

We’re thrilled to announce a major milestone for the Saint Elizabeth Foundation | SE Health: our new partnership with Hospice UK.

At the announcement event, the room was absolutely jam-packed, and the energy was electric, a powerful reflection of the excitement surrounding this collaboration and the shared belief in what we can achieve together.

For years, we’ve been learning from communities across Canada, especially through Journey Home Hospice, our specialized program for people experiencing homelessness and other structural vulnerabilities. This work helped shape our Canadian Centre for Equity and Innovation in End-of-Life Care, created to re-imagine hospice through compassion, dignity, and equity.

But the challenges we face in Canada are not unique. Around the world, hospice leaders are asking the same urgent questions:

– How do we reach people excluded by traditional models?

– How do we adapt hospice for complexity, trauma, and inequity?

– How do we ensure dignity and care for everyone, everywhere?

That’s why this partnership matters.

🌟 Together with Hospice UK, we’re sharing innovations, challenging assumptions, and co-creating models of care that ensure no one is left behind, anywhere.

🌟 This collaboration strengthens an international movement toward more inclusive, community-rooted, and equitable hospice care.

🌟 It reflects our shared commitment to re-imagining what end-of-life care can and should be.

We’re honoured to work alongside an organization internationally recognized for its leadership and bold vision.

What an incredible experience for the Saint Elizabeth Foundation at the Hospice UK Conference 2025!

Our Executive Director of Journey Home Hospice, Fr. Dr. Matthew Durham, and SE Health’s Chief Operating Officer, Nancy Lefebre, were honoured to present the Saint Elizabeth Foundation | SE Health Model of Care and the work of the Canadian Centre for Equity & Innovation in End-of-Life Care, both shaped by lived experience supporting patients and families through our specialized program, Journey Home Hospice.

The room was filled to standing-room-only, with remarkable curiosity, enthusiasm, and genuine commitment to advancing equity in palliative and end-of-life care. We were energized by the meaningful conversations, the shared passion for innovation, and the incredible work happening across the UK.

We’re leaving inspired, grateful, and excited for the possibilities ahead as we continue strengthening partnerships and championing equitable care for those most often left behind.
