With the FHO+ model underway, one key enhancement physicians should know about is the increase to in-basket compensation.

In-basket services make up a major portion of FHO billing, and under FHO+, shadow billing is increasing from 19.4% to 30%. This applies to all in-basket services, including special visit premiums, and represents a meaningful boost in revenue for the day-to-day care you provide.

It’s also important to remember that the fee-for-service hard cap remains measured at the group level.

The new FHO+ model brings important enhancements to how family physicians are compensated, including updated funding streams and new patient attachment incentives designed to improve access to care.

Under this model:
- Established physicians will receive $100–$180 per new rostered patient, based on RIO score and patient age.
- New graduates will receive $150–$270 per patient to help build their roster and support early career growth.

These bonuses are effective from July 1st 2025, and more information from the OMA will be released on how and when these bonuses will be paid out.
